Faith-based family health care center seeking a Triage Nurse to join our growing team of medical professionals! The Triage Nurse performs telephonic care coordination using critical thinking, assessment skills and established clinical protocols to assess the severity of health concerns. The Triage Nurse will assess patient's condition and appropriately provide follow up guidance which includes scheduling appointments, following up with providers or directing patients for emergency care. This position offers a $2,500 bonus at the completion of the first and second year of tenure. Positions are full time at 40 hours per week. Qualified candidates who are bilingual in Spanish and English are encouraged to apply. **covid and influenza vaccines are required** QUALIFICATIONS: RN, current Pennsylvania license, BSN preferred. Current nursing license in good standing. Pediatrics or OB experience a plus. 3+ years of experience as an RN utilizing progressively complex clinical judgment and assessment. KEY JOB RESPONSIBILITIES: Manages the Nurse Phone Line: nurse-directed patient telephone calls, same day and urgent appointments request, and all other health care services request. Uses clinical judgment to determine if necessary emergency response is required. Uses clinical judgment to determine if necessary emergency response is required. Evaluates and assesses patient's health care needs based on patient's signs and symptoms utilizing established triage protocols and guidelines Utilizes clinical judgment based on scope of practice as well as obtains appropriate input and direction from the provider as needed to determine best clinical course of action and recommendations for specific patient health care needs. Accurately documents symptoms/complaints, nursing assessment, advice provided and patient/caller response. Coordinates and schedules same-day/ urgent, and follow up appointments on the provider schedules Utilizes standing orders, triage protocols and algorithms appropriately Manages prescription refills per protocol. Provides patient education: e.g... chronic disease, preventive and family. Must have a desire to serve the underserved in the East Liberty/Lincoln-Lemington areas of Pittsburgh. Compensation: Commensurate with experience.
