POSITION SUMMARY Provides support to the Managers of Foster Care, Foster Care Team Supervisor, and Foster Care Case Managers as assigned by the Manager. The Team Permanency Specialist will assume primary responsibility for information management, including records procurement, management and tracking, development and maintenance of tickler systems, collating documentation for critical case events, and creating and maintaining the team's calendar of staffings, court hearings, Administrative Case Reviews and other critical case events as required. The Team Permanency Specialist will provide supplemental client transportation as assigned by the Manager. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES Review and ensure all court orders have complete information checked and documented. Scan and enter court orders in Salesforce. Create monthly reports of court hearing due dates. E-file court reports and track reports submission of court reports. Alert supervisor of any overdue report. Review and ensure completeness of records for any parent felony, child medical issues, mental health records/psychiatric for parent or child. Identify any relative/social supports mentioned in the history, integrated assessments, etc. Coordinate with the case manager of biological and child service providers to request quarterly reports or more as needed. Scan and enter documents into salesforce. Create monthly reports for workers and supervisors for provider reports submission. While reviewing medical records, maintain a current copy of the child's physical, dental, and vision, along with other pertinent health information identified, and enter it into Salesforce along with a copy. Create monthly reports for workers' and supervisors' health record tracking. Maintain schedule of team's CFTM through Salesforce, and create a monthly report of completed that month and due for the next month to worker and supervisor Maintain information regarding the team's foster parent use of daycare. Assist in gathering and submitting paperwork to ensure the timeliness of daycare approval. Document approval dates in Salesforce. Create monthly reports for workers and supervisor day care coverage/application status. Gather required documents for ACR, CIPP, Legal Screening, and/or other events that require paperwork submission. Review case files for completeness of required documents, and coordinate with the case manager/supervisor request for missing information. Once a supervisor has deemed a legal screening packet due date, notify the Family Finding worker and Licensing Supervisor that a 411 is needed on the current foster family. Assist in completing ICPC documentation Other duties as assigned. SKILLS and ABILITIES Possess the experience, personal qualities, and practice skills to work effectively with professionals from various disciplines, both internally and with other organizations. Ability and willingness to adopt the agency practice model, CARE, and pass TCI tests. Strong organizational and time management skills. Excellent oral and written communication skills. Excellent interpersonal skills. Must be able to drive independently. EDUCATION AND/OR EXPERIENCE A High School diploma or GED is required; an Associate's or Bachelor's Degree is preferred. A minimum of one year working with children. Must be at least 21 years of age Must possess and maintain a valid driver's license Must possess and maintain proof of insurance CARE & TCI One of the essential job functions of any position within the agency is to align your performance with our overarching philosophy of CARE. All decisions, actions, communication, and performance should align with the six CARE principles (Developmentally Focused, Family Involved, Relationship Based, Ecologically Oriented, Competence Centered, and Trauma Informed). Not only must you complete the CARE training with satisfactory participation, but you must then put that training into practice in your everyday work. Every employee must also pass TCI training with a satisfactory grade in written and/or physical testing. All employees must remain current on refresher training that is provided monthly at all locations. You will be tested every 6 months on the physical portion, if applicable to your position. You will be tested annually on the written portion, regardless of your position. If you are not current on your TCI refresher training every six months, you will be required to retake the full course. If you do not pass either test, you may be required to retake the full course.
