OVERVIEW Boston Senior Home Care is a private, nonprofit organization that provides case management, person-centered care planning, home care and long term services and supports to those who wish to remain in the community but require assistance to do so. One of 26 Aging Services Access Points (ASAPs) in Massachusetts, Boston Senior Home Care is committed to providing the best assistance possible for those in need. Although the majority of the people we assist are over age 60, we offer long term services and supports for children and young adults. We also provide programs and education initiatives for those serving as caregivers and information about maintaining a healthy lifestyle and controlling chronic disease at any age. JOB SUMMARY The Supportive Housing Wellness Nurse / CAE Nurse is a unique position to support a dual program initiative. This full time position (35hr) divided into appropriate hours to meet the needs of each program. The RN functions as part of the interdisciplinary Supportive Housing team and participates in wellness planning activities for all supportive housing residents working with the interdisciplinary supportive housing team to: Improve health management through education and support. Increase knowledge of relevant wellness topics such as nutrition and common medical diagnoses. The Registered Nurse will support the Home Care program (CAE) and function in collaboration with the case management team in direct care service planning for Home Care consumers. Responsibilities include in-home clinical assessments of consumers to determine personal care needs and medical eligibility for programs funded by the Executive Office of Elder Affairs (EOEA) and the Division of Medical Assistance (DMA). The nurse also performs in-home nursing assessments to determine the personal care needs of Home Care consumers. The incumbent will need to be flexible to work where the leadership requests the support depending on program's needs. AN IDEAL CANDIDATE WOULD HAVE: Must have a Bachelor's degree in Nursing or Human Services. Supportive housing or home care experience preferred. A valid Registered Nurse's license from the Commonwealth of Massachusetts and a Class 3 Motor Vehicle license. Strong assessment, independent caseload management, and interpersonal skills. COMPETENCIES Knowledge of the mental health, elderly, developmentally challenged and disabled populations. Strong assessment, independent caseload management, and interpersonal skills.
