Major Performance Duties Conducts all business in accordance with agency policies and procedures, Fair Housing, Americans with Disabilities Act, Fair Credit Reporting Act and all other laws pertaining to rental housing and supportive housing programs. While the following should be considered a comprehensive description of this position, it should also be noted that some responsibilities and duties may not be specifically addressed. Case Manager Establish a trusting relationship with individuals/families to promote their well-being and housing stability. Maintain case responsibility for individuals/families who are assigned to caseload in your designated program. Provide case management services as appropriate to each individual/family's acuity level and case specific needs. Establish outcome objectives for interventions and monitor progress towards goal achievement. Coordinate services and monitor progress on individual/family referrals and linkages to community resources and other service providers. Facilitate coordination of the integrated case plans with the adult protective services or child welfare system, as appropriate. Link individuals/family members to appropriate primary care. Link individuals/family members to appropriate substance abuse and/or mental health treatment facilities. Assist and support individual/family members with drug and alcohol plans to prevent relapse. Provide individuals/families with referrals to medical and mental health services. Assist individuals/families with housing searches and applications when needed. Assist individuals/families with completion of job-related services such as resumes, application completion and searches and/or refer to available supportive services programs. Assist individuals/families with transportation or bus passes (when available) to appointments necessary to establish services (i.e., medical, mental health and benefits) Provide coordination of academic support; meet with CMS social workers and other staff; and provide support with Individual Education Plans. Monitor and provide assistance as needed. Monitor academic achievements, promotions to the next grade level and school attendance. Assist individuals/families in establishing goals and assessing barriers to achieving goals. Establish outcome objectives for interventions and monitor progress towards goal achievement. Provide situation-specific skills training as needed. Participate in individual/family consultations and team meetings. Complete housing visits and visual inspections at least monthly. Assist the Housing Coordinator in building landlord relationships. Assist housing residents with establishment of relationships with landlords and utility providers. Maintain documentation according to HMIS and agency policy. Case management notes must be entered within 24-48 hours of encounter. Sign all notes in HMIS and document follow-up appointment dates with clients. Maintain an appointment calendar in Outlook or similar system. Complete daily activity reports by 9:00 am on next business day. Maintain adequate mileage logs and submit to your Program Coordinator for approval no later than the 5th of each month. Properly report PTO requests. Perform other duties as requested by the Manager of Program Service or the Chief Operating Officer within the scope of the job. Uphold the Supportive Housing Communities Policies and Procedures at all times. Uphold the Supportive Housing Communities Code of Ethics and the Social Work Code of Ethics at all times. Community and Resident Relations Maintain a positive customer relations attitude with all stakeholders. Communicate effectively both orally and in writing. Maintain awareness of public perception of Supportive Housing Communities and contributes ideas to the Manager of Program Services and Chief Operating Officer for improving public image of the organization, its mission and activities and resident and community satisfaction. Marketing and Development Must be knowledgeable of all phases of leasing and resident retention. Assist with volunteer projects for support services (e.g., Thanksgiving and Christmas distributions, intern activities, etc.) Maintain awareness of the community environment and assist the Chief Executive Officer in improving community satisfaction. Mental Health Access to Treatment and Support (Licensed Clinical Social Workers only) Must have a license in the state of North Carolina (fully licensed or provisional) Maintain positive customer relations attitude. Perform comprehensive assessments for mental health, substance abuse, co-occurring disorders, domestic violence, and medical needs. Develop treatment plans and evaluate client progress. Coordinate and facilitate support team meetings with client and community providers. Provide individual psychotherapy for identified and self-referred individuals. Refer and link clients to appropriate resources and supports. Participate in supervision and/or consultation ongoing or as needed from a Licensed Clinical Social Worker. Promote, attend, and support positive community building, advocacy and other community relations activities as assigned by the Director of Program Services Participate in ongoing continued education and activities to maintain licensure. Position Requirements Commitment to the mission of Supportive Housing Communities. Bachelor's degree in social work or related field, master's degree in social work preferred and two years of relevant work experience. LCSW is preferred; LCSW-A will be considered with appropriate supervision/ Must have a valid driver's license. Must be able to work independently. Must be able to communicate well with individuals/families and community service providers. Must have a working knowledge of community resources and conflict mediation. Must be familiar with mental health and substance abuse commitment laws and procedures Must be able to intervene in and resolve crisis situations. Demonstrate good judgment under pressure. Utilize community resources necessary for conflict management and/or services. Adhere to confidentiality laws. Ability to comply with policies, rules, and procedures of the organization. Ability to frequently lift or move 10 lbs. and occasionally lift 25 lbs.
