ICRS Assistant Support Worker - Leicester Driving licence and access to own vehicle essential£13.93 per hour Contract – Full Time Duties/Responsibilities: Identifying and correcting discrepancies within information received and tailoring the delivery of care to the needs of the individual. Acting as the first responder to someone that has fallen, risk backssing the situation and making decisions on how the person needs to be supported including, the requirement for, medical attention and referrals for further support. Monitoring and accessing the functional abilities of individual’s, amending their package of care accordingly and referring them for further support where required. Encouraging, motivating, and empowering individuals to meet their goals as part of their Reablement/ICRS care plan. Undertaking a range of activities which enable individuals to live independently including but not limited to, personal care, shopping, cleaning, cooking, and gardening. Completing and reporting clinical observations blood pressure, oxygen levels and temperature readings and promptly identifying when a person’s health is declining. Identifying where assistive technology/telecare devices may be of benefit to service users and arranging for their provision. Completing daily records which accurately reflect the interventions delivered and progress of the individual.
