Support Coordinator-Community Health Full Time The Parker Jewish Institute for Healthcare and Rehabilitation, located in New Hyde Park, NY, is seeking Support Coordinators with strong customer service skills to join our team and play a key role in the administrative function of Parker's Community Health Services Program-encompassing Parker's Certified Home Health Agency and Comprehensive Community Hospice. In this position, the Community Connector-Support Coordinator is responsible for providing non-clinical support to the Care Management team such as providing telephonic outreach to our members, providers and community based organizations, assisting with questions and support such as locating service providers/vendors for our members, and other key clerical support duties that allow us to provide exceptional quality care to the members we serve. Position Qualifications: High School Diploma required; college degree in a health related field a plus Bilingual in English and the following language a plus: Korean Prior experience working in a home care or other healthcare setting strongly preferred Prior experience/proficiency in NetSmart or another EMR (electronic medical record) a plus Excellent communication and follow-up skills Fully vaccinated against COVID-19 SUMMARY: This position is a key administrative role on our Home Care/Hospice team, with the ability to make an impact and offers a significant personal and professional opportunity to join a world-renowned health care organization.
