With increased focus on preventing evictions and providing long-term stability for those who are establishing their independence, we believe a true systems change is possible. We imagine a world where the need for shelter is minimal, because individuals and families are able to stay in their homes instead of being evicted and those who are struggling to make ends meet can get the help they need to maintain stability. We inspire lasting independence for all people affected by homelessness and poverty through a continuum of housing and support, using the most innovative, progressive, equitable and inclusive methods. As part of a collaborative five-year project with Yale New Haven Health, New Reach is seeking a full-time Social Worker to provide person-centered case management to women (including youth) who are affected by homelessness and identified as having a co-occurring disorder to ensure follow-up of recommended treatment and stabilize housing. This role will help empower clients to gain recovery and resiliency through phased case management and will build a network of accessible and appropriate support services for individual participants that can range from family and neighborhood groups to mental health therapy, substance abuse treatment, or holistic care such as nutritional services. Services are voluntary and typically occur in the individual's home (if housed) or a community location of the client's choice. The Social Worker The Social Worker will be responsible for engaging and enrolling individuals who are chronically homeless who require treatment and recovery support services in preparation of entering permanent housing. The Social Worker will p rovide case management with an average weighted caseload of homeless women via Critical Time Intervention to link with and retain clients in housing and other necessary services and a ct as liaison to community/state agencies in developing and implementation care plans for clients. The Social Worker will m aintain ongoing client files, including person centered service plans in collaboration with case workers, correspondence, and case notes. The Social Worker will a ssist in program development and quality assurance activities as directed by supervisor and e nsure that those who need mainstream benefit programs (e.g., SSI/SSDI, TANF, SNAP, etc.) are referred to the SOAR worker. They will f acilitate meetings related to ICP program as needed in the absence of the Clinical Director and ICP lead social worker including but not limited to CTI Team Supervision, ICP Team Meetings, Clinical Group Supervision, etc. The Social Worker will a ssist clients in identifying unaddressed needs related to benefits, education, housing, medical and other needs and a ttend community meetings as necessary to represent the ICP team, including Community Care Team Meetings, CAN Housing Case Conference, and other meetings when appropriate. The Social Worker will also p rovide support and maintain regular communication with all ICP team members. A qualified Social Worker will have a Masters Degree (MSW preferred, MFT or MS in Counseling will be considered) and will preferably have licensure or license eligible within 6 months of hire. A qualified Social Worker will need experience in Social Services and be highly motivated, creative and have excellent interpersonal skills. The Social Worker will be able to provide comprehensive, intensive services to the homeless, including acting as a strong advocate. New Reach team members are expected to have a solutions based approach to presented problems and be supportive, engaging and encouraging to all peers and staff. New Reach team members are adaptable to change and willing to take on responsibility. The hours for this position are Monday through Friday, 8:30-5. The pay for this role is $22-$25 an hour.
