POSITION SUMMARY: Assists patients and their families in dealing with social and emotional functioning, under supervision of Medical Social Worker. Provides biopsychosocial assessment, crisis intervention, counseling and linkage with resources for Home Health and Hospice patients. MINIMUM OF QUALIFICATIONS: Education: Master' degree in social work (MSW) from a school of social work accredited by the Counsil on Social Work Education. CPR certification required. Valid driver's license. Experience: One year of experience in social work in a health care setting. Job Knowledge: General knowledge of Medicare requirements for home health and hospice. Skills: Ability to demonstrate professionalism and compassion while providing efficient services for home patients during often emotionally difficult circumstances for patients and/or families. Ability to communicate well with patient, family and other staff involved in patient's care. Must work with people in a tactful, diplomatic and professional manner. Must be able to remain pleasant and patient, and have good communication skills. Computer proficiency required. Additionally, must have the skills to write case notes and business correspondence. Must have the ability to demonstrate effective time management skills while working independently with minimal supervision. Personal Protective Must be able and willing to wear appropriate Personal Protective Equipment : Equipment (PPE) required to perform the job safely. Travel : Travel is required in our service area using company vehicle. Occasional travel may be required for CEU training and certifications in and outside of Lake County. May also be required to provide client transportation on occasion. Expected Hours of Work : Hours of typical operations are M-F, 8:30am to 5:00pm. Must be able to work a flexible schedule, as needed.
