Mozaic (a chapter of The Arc) is hiring for a Social Worker- Fee for Service Social Worker (LMSW/LCSW) Duties and Responsibilities: Provides social work services in an individual or group setting to individuals with disabilities. Provides services to enhance individual's quality of life and enable the person to live independently or as independently as possible in a family/guardian household or placement setting. Develops and conducts small groups on topics such as sexuality, sexual abuse prevention, and rights of individuals. Acts as an advocate for individuals served to ensure their rights and needs are met. Serves as a liaison between clients, agency programs, family/guardians, and other agencies to promote communication and integration of program services. Documents individual's goals and progress according to agency policy. Attains and maintains Personal Outcome Measures (POMs) interviewer certification and conducts POMs interviews as needed. Are you qualified? Master's degree in social work. Licensed and currently registered as a Licensed Master Social Worker by the NYS Dept of Education Experience facilitating groups and providing services to individual's families or case management. One year of experience working with individuals with developmental disabilities preferred. Ability to communicate and work effectively within a team environment. Ability to interact with staff and individuals in a professional and friendly manner.
