Looking for a Licensed Social Worker for our Marshwood Center 32 hours per week/DAYS/BENEFITS APPLY TODAY! Enrich lives including your own as a nursing center Social Worker where you will provide services for our patients/residents, that render the highest practical level of physical, mental, and psychosocial well-being and quality of life. Position Highlights.Identify through communication with patients/residents, their family members/significant others, and nursing center staff, patient/resident's strengths, social, emotional, and mental health needs' and then provide, develop, and/or aide them in accessing services to meet their needs Collaborate with the interdisciplinary team to promote and protect resident rights and the well-being of all patients/residents. This includes educating staff on the role of the social services Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs Hold meetings with patients/residents, families, and significant others as requested and to provide support and information so as to improve quality of life Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers Establish relationships and maintain contact and referral flow with community based agencies/services that benefit our patient/resident population. We take great pride in meeting or exceeding CDC and CMS standards Bachelor's degree from accredited school of Social Work or related field is required Must possess any certifications/licensures as required by the state of employment to practice in long-term care One to three years of supervised social work experience in a health care setting, working directly with individuals, is preferred Additional certification such as Geriatric Case Management, Hospice & Palliative Care, Gerontology, Clinical Social Work, Health Care, Nephrology, Mental Health, and/or Substance Abuse is preferred.
