The Brudnick Center for Living is a CMS rated 5-star nursing facility offering short-term rehabilitation, traditional long-term care, and memory care with an outstanding team of professionals who go above-and-beyond each day in the care of our residents. Job Description Social Work/Case Manager assesses the needs of the patient and the patient's family, when appropriate and arranges, coordinates, monitors, evaluates, and advocates for the patient during their subacute rehab stay at Brudnick Center for Living. The Social Work/Case Manager is responsible for fostering a climate, policies and routines that enable patients to maximize their independence and dignity. This climate shall provide patients with the highest practical level of physical, mental and psychosocial well-being and quality of life. The presence of a Social Work/Case Manager addresses a patients non-medical concerns allowing other disciplines on the team to focus on their specific areas of expertise. Responsibilities include but not limited to: Identifies patient's /family's psychosocial concerns related to obstacles t hat may impact the ability to create a timely safe discharge plan back to the community. Provides supportive short-term counseling for patient/family as needed. Maintains effective working relationships with medical and therapy teams to work collaboratively to provide for ongoing care needs of the patient. Ensures Health Care Proxy is present upon admission and if not educate the need to have HCP initiated. Educates patients and families/significant others regarding their rights and responsibilities, effective problem solving and the extent of community services that are available to them to create an effective discharge plan. Coordinates the patient discharge planning process and makes referrals to appropriate Home Care/VNA agencies prior to the patient's return to the community. Participates in the development of a written, interdisciplinary plan of care for each patient that identifies the psychosocial needs/issues of the patient, the goals to be accomplished and the appropriate Social Work/Case Management Interventions. Review proactively specific needs that a patient may have upon discharge such as oxygen or any home equipment needs . Qualifications Massachusetts Social Work License (LCSW or LICSW) LSW Academic credentials as mandated by current state licensure requirements. Previous experience in acute or sub-acute setting preferred.
