When a loved one comes to our skilled nursing facility, they will be well cared for, will feel heard, and will know that they matter. We strongly believe that the key to this is through our incredible staff! What you will do: The Social Services Director is responsible for providing services to meet the social and emotional needs of the residents/patients. Develops a rapport with the residents/patients and presents their concerns to center management. Charts as assigned Assists with Resident Council Spearheads all discharge planning for residents Follows all center's policies and procedures Qualifications: Bachelors degree in Social Work REQUIRED Qualified Social Worker PREFERRED Ability to speak, understand, write and read in English Previous experience in a long term care setting PREFERRED Ability to pass a background screening with no disqualifying events Caldera Cares for you!
