Vasona Creek Healthcare Center a premier skilled nursing facility located in Los Gatos, is looking for a Social Services Assistant/clerk/transporter who can work in a fast-paced environment, provide excellent patient care, have a positive attitude, and work together as part of a team. FULL TIME Responsibilities: Establishes course of action by exploring options; setting goals with resident and their families. Assist residents in achieving the highest practicable level of self-care, independence and well-being. Provide medically related social services so that the highest practicable physical, mental and psychosocial wellbeing of each resident is attained or maintained. Collect patient data upon referrals from referral sources.
