This opportunity is with our Good Shepherd - Emmanuel House How You Will Make an Impact The Clinical Intake Coordinator position at Good Shepherd Centres plays a crucial role in upholding the organization's mission, vision, and values through exemplary leadership, role modeling, and service delivery. This role involves managing and coordinating the referral, intake, and discharge processes for the Seniors' Programs and Emmanuel House Hospice, ensuring services are delivered in a dignified, culturally-competent, and anti-oppressive manner. You will collaborate closely with program directors, community partners, and family members to keep all stakeholders informed and up to date about the services provided. You will maintain accurate data related to all aspects of referrals, admissions, and discharges. Responsibilities include effective communication with both internal and external stakeholders to facilitate smooth transitions and support for clients and staff. You will also play a key role in crisis response, participate in leadership meetings, and assist in the recruitment and training of clinical staff. Additional duties include after-hours/on-call support for urgent care and operational issues, and active participation in both internal and partner agency meetings...
