A School-Based Case Manager with Encompass Christian Counseling partners with school officials to provide support and case management services to students and families at assigned schools. This position is located in the Waynedale School District, in Wayne County, Ohio. Rewards : $1500 Employment bonus - paid within 60 days of employment 401(k) with employer match CEUs Requirements : High school diploma or equivalent required. Bachelor's degree preferred with strong preference for degree in Social Work or Psychology. 3 years experience in mental health or related field. Experience working with children and families strongly preferred. Responsibilities : Adhere to CCHO's statement of faith and code of conduct. Treat others with Christ-like attitude, while helping them experience their worth in Christ, and demonstrating behaviors reflecting CCHO's core values of Relentless Commitment, Selflessness, and Kindness. TEAM first. Provides case management services that are focused on the client's ability to succeed in the community; identify and access needed services; show improvement in school, work, and family as well as integration and contribution within the community. Ongoing assessment of client needs, assists in achieving personal independence, facilitates development of daily living skills, coordinates and assists in crisis management. Advocacy and outreach; education and training for client and family. Activities that increase the client's capacity to positively impact their environment. Completes or assists with treatment plans (ITPs). Provide on-call services, as needed.
