RN Medication Pass

Description:
Urgent opening to cover medication passes two times per day. Open shifts 7 days a week 7am to 9am and 7pm to 9pm Nurse must hold a current Registered Nurse (RN) licensure in good standing with the State of New Mexico. Medication pass in ICF-ID home. Must be reliable, professional and have training or past experience working with adults with Intellectual Disabilities. All nursing services provided must be completed in according the New Mexico Medicaid Home and Community Based Waivers and long-term care facilities, New Mexico Board of Nursing and policies, insurers, procedures and practices of Great Livin' LLC. All services provided to Great Livin' clients shall be performed according to the development of client plans of care / individual service plan and all requirements of any applicable federal and state laws, regulations, standards, and policies. The Nurse shall enter all nursing information into THERAP (State required electronic health record) as required by New Mexico Medicaid Home and Community Based Waivers and rules, regulations, laws and standards GL', LLC operates under. 1. Assess, monitor, and assist with Individual's health & medical needs 2. Administer Medications and Treatments as indicated on MAR, ordered by medical director. 3. Availability to give PRN medications when needed is highly prefered. MEDICAL ON-CALL, SUPPORT TO COACHES FOR MEDICAL DECISION MAKING (PRN, MED ERROR, TRIAGE) · Implement the Great Livin' nursing on-call system · Ensure medical on-call 24/7 coverage to address medical issues including, but not limited to triage support and requests for approval of PRN medication · Logging on-call calls at time of phone call/ ensuring documentation is complete in Great Livin' designated systems · Follow-up on on-call recommendations as needed including, but not limited to additional doctor's appointments and notifications and crossover on-call with Head Nurse at the end of the on-call shift Additional Tasks · Attend Management Team and QA/QI Meetings as requested (backup to Head Nurse as needed) · Other tasks as assigned by the Great Livin' LLC Directors relevant and within the guidelines mission and ethics of Great Livin' LLC · Completion of job-related requirements per Great Livin's Policies and Procedures, and State Standards, Procedures and expectations (including - as applicable, but not limited to: completion of all required documentation before leaving shift; completion of State Abuse, Neglect and Exploitation Reporting; GERs; Time IPS) ·Back up for administering medications during weekday and weekend (as needed) Knowledge and skills: · Must be able to speak, read and write in English · Proficient in use of a personal computer and various software packages · Must have working knowledge of Therap, and other agency systems as applicable · Knowledge of agency positions and responsibilities, and who to go to with specific questions and/or concerns · Must be of good integrity and possess adequate physical, mental and emotional stability so as to meet job duties in a safe and responsible manner · Must have a working knowledge of client funding rules and regulations (DD Waiver, Mi Via), and required documentation for Medicaid billing procedures as related to nursing services · A general understanding of the DD Waiver/Mi Via/ICF Standards and a working knowledge of Supported Living, Adult Nursing, and Customized Community Supports Standards · Able to establish priorities, work independently, and proceed with objectives without hands-on supervision · Able to handle and resolve recurring problems · Willing to work the hours necessary to meet the needs of the individuals with whom you work Experience / Educational Requirements: · Register Nurse in good standing with the New Mexico Nursing Board · Complete required trainings and certifications including, but not limited to, those mandated by the DD Waiver, Nursing Board, State and Federal Standards and Great Livin' LLC · Agreeable to additional training(s) when appropriate for the enhancement of position skills Expected Job Outcomes · Individual's health and medical needs are well addressed · Timely filing of necessary paperwork and reports · Concise and accurate completion of assigned tasks · Conduct himself/herself as a member of the Great Livin' team · Assist with the growth, quality and direction of Great Livin' · Cohesive communication between Great Livin' Head Nurse, Program Director, Operations Director, Chief Operations Officer, relevant team members related to Great Livin' and the people we support
