The Resident Social Worker works with Independent Living residents and the Ambassador Program to make meaningful, valuable contacts with residents, families, and community professionals in order to facilitate services for the residents.Key Responsibilities: Enhance new resident orientation for acclimation into Butterfield Trail Village Facilitate new residents services while residents continue to reside in Independent Living Collect new resident's data into Matrix Care. This includes but is not limited to: medical information, emergency contacts, advanced directives, etc. Refer resident (s) to appropriate resources including but not limited to: wellness, UAMS, dietary, therapy, social, health and community agencies as appropriate. Develop relationships with resident(s) families to ensure effective communication regarding resident's services, needs, and the transition process. Coordinate educational opportunities for residents and families to learn about the transition process to Assisted Living (AL), Healthcare Center (HCC), and Special Care Center(SCC) Conduct extensive pre-admission visits with Independent Living (IL) residents and their families as they make decisions about transitioning to the Health Care Center (HCC), Special Care Center (SCC) or Assisted Living (AL). Explain and educate on the transition process regarding what to expect. Coordinate with Health Care Leadership as appropriate. Assist the Senior Director of Resident Services in completing Life History Assessments with residents in Independent Living. Document interactions with residents and families in Matrix Care. Other duties as assigned Minimum Qualifications: Bachelor's degree in social work and a valid, current Arkansas social work license Minimum of 12 months of social work experience Excellent verbal and written communication skills in both one- on-one and in group settings Excellent interpersonal and customer service skills Excellent organizational skills and attention to detail Ability and willingness to work with social perceptiveness, empathy and compassion Preferred Qualifications: Master's Degree 3+ years of social work experience Prior experience in a retirement setting Job Posted by ApplicantPro
