Job Summary: The incumbent in this position is responsible for providing twenty-four hour supervision to residents of a supervised living environment under the extensive advisement and consultation of the Site Supervisor. Duties are carried out in accordance with general work instructions and established practices, policies and procedures; however, the incumbent must make independent decisions concerning the safety and well-being of the residents. The incumbent is on call for the caseload of thirteen residents and urgent facility needs, sharing on call responsibilities with the Site Supervisor and Senior Counselor. Nature and Scope: Work is performed under the general supervision of the Site Supervisor and at times, the incumbent takes direction from the Director of Residential Services. The incumbent interacts with staff and residents on a daily basis and frequently interacts with community providers via telephone, email and in person contacts. The incumbent has access to confidential information as necessary to perform job duties. Adherence to confidentiality policies, and positive and effective interpersonal/communication skills, are required and a high priority in this position. In the performance of the incumbent duties as an employee of Greenwood Residences, Inc., the incumbent is expected to respect the right to privacy and security of Protected Health Information (PHI) or Electronic Protected Health Information (EPHI) of individuals receiving services and their families. Major Tasks, Duties and Responsibilities : (*connotes Essential Job Functions Note: “Essential functions” are primarily job duties that the incumbent must be able to perform unassisted or with some reasonable accommodations. Teaches, assists and monitors health needs of the residents. Coordinates and assists residents with obtaining all routine and urgent health care related needs, including transporting residents to appointments. Assists residents with understanding medical issues, following care plans and monitoring progress. Formulates resident service plans by identifying goals, objectives and plans of action in conjunction with the resident. Writes plans that are behavioral in nature, clearly stated and reflect principles of rehabilitation and based on resident’s needs/strengths, values/preferences, hopes, dreams and aspirations. Meets with residents on a regular basis to monitor performance and develop new objectives and activities. Ensures service plans, resident case records and unit paperwork are current, accurate, legible and in compliance with agency, Office of Mental Health (OMH) and Medicaid regulatory standards. Completes all paperwork in an accurate and timely manner. Effectively manages crisis or emergency situations by responding immediately in a therapeutic manner while following established agency and regulatory standards independently or with consultation from the Site Supervisor and Senior Counselor. Effectively assists Resident Counselors in managing crisis as part of on-call responsibilities. Completes an accurate and concise written report of an incident. Supervises resident medication(s) to ensure that residents medicate as prescribed. Monitors prescription expiration dates and orders refills to ensure resident medications are available in the unit. Encourages residents to gain skills necessary to medicate more independently. Consults with medical and mental health staff regarding both medical and psychiatric care. Assesses resident level of functioning, skills and rehabilitative counseling needs in the area of daily living skills. Teaches, assists and monitors self-care skills in the area of personal management. Assesses resident clothing and hygiene supply needs and coordinates the purchase of items. Teaches, assists and monitors residents in the area of nutritional management. Coordinates nutritional needs and establishes resources as needed. Ensures that meal preparation is complete and kitchen areas are clean and safe. Teaches, assists and monitors residents in the area of money management by providing basic budgeting skills. Coordinates collection of monthly SSI rent & services fee payments and supervises Resident Trust deposits and withdrawals in the management of resident entitlements. Complete recertification applications as needed. Assists residents with applying for and maintaining all benefits. Teaches, assists and monitors resident’s home management skills by coordinating or providing assistance to clean resident bedrooms and other general areas of the site. Teaches, assists and monitors residents in time management. Coordinates the planning and implement of group social recreational activities. Plans and implements activities that reflect the culture and preferences of the residents. Teaches, monitors and assists residents in community life. Transports and assists residents as needed for the utilization of community resources. Ensure building security and safety at all times. Teaches, monitors and assists resident in safety management. Ensures that fire drills are conducted minimally on a monthly basis. Ensures residents possess the knowledge of site evacuation plans and procedures. Provides direction and training and conducts resident self-preservation test. Maintains physical plant and monitors resident behaviors for identifying potential hazards. Teaches and assists residents in problem solving skills. Allows residents to express their needs and values without imposing personal values. Presents concerns and ideas in a professional manner and serves as a role model to residents and co-workers. Monitors maintenance needs and requests maintenance/repairs. Assists in maintaining the appearance of physical site and grounds. Maintains a neat, clean clutter free office. Cooperates and coordinates activities and services with agency staff. Accurately utilizes written and oral communications to keep team members advised of plans, program concerns and significant resident behaviors. Is prepared for and attends staff meetings. Performs additional duties such as coverage (working additional hours to ensure quality care & services) as assigned by the Site Supervisor. In a timely fashion, accurately completes all required paperwork. Participates in workshops, seminars and in-services. Attends all agency mandatory training sessions and a general amount of scheduled in-services. Participates in advisory meetings to review program and identify career goals and objectives with Site Supervisor. Completes additional task assigned by the Site Supervisor and Director of Residential Services. Performs essential functions of the Site Supervisor in their absence. Job Requirements: (all essential functions) Note: Occasionally = if the activity is required 1 - 33% of the time Frequently = if the activity is required 34 - 66% of the time Constantly = if the activity is required greater than 66% of the time Able to maneuver constantly throughout the working environment. Able to communicate and interact positively with a variety of individuals from various socio-cultural and economic backgrounds constantly. Able to manage resident and unit needs whether working individually or with coworkers constantly. Able to respond to individuals’ spoken needs constantly. Able to read, and understand/interpret a variety of correspondence, written materials, data, and policies and procedures frequently. Able to lift 20lbs occasionally. Able to open/close either vertical or lateral filing cabinets, desk drawers, etc. on a frequent basis. Able to concentrate on detailed and complicated matters constantly with occasional interruptions; attention to detail is critical in this position. Able to comprehend oral instructions and remember the same in detail constantly. Able to cope with and work in a frequently fast paced and occasionally high stress work environment. MINIMUM QUALIFICATIONS Valid NYS Driver’s License requirement Educational Requirements : BS, BA in Mental Health or Human Services field of study. Candidates without a BA/BS degree, with 1 year of experience in the residential mental health field, will be considered. Experience : Six months direct service in Mental Health or Human Services setting.
