Registered Nurse-Care Manager Full Time Bilingual Korean Parker Jewish Institute's Certified Home Health Agency is seeking a dedicated and experienced RN-Care Manager to play a key clinical role on our Care Management interdisciplinary team. This is a telephonic case management role-remote opportunities available for qualified candidates! Parker Jewish Institute's Certified Home Health Agency is seeking an RN, responsible for the psychosocial needs and services of members across the continuum of care. This position will interview, coordinate and plan activities and behavioral routines to meet the social and medical needs of Members and their families. The RN Care Manager will assume a behavioral health leadership role within the interdisciplinary team to achieve optimal clinical and resources outcomes. This is an excellent clinical opportunity for a self-motivated and team oriented Nurse looking to make a difference in the lives of others through coordinating quality care! Position Qualifications: Must be a Licensed RN in New York State 2-4 years relevant Nursing and/or Health Care experience with client coordination responsibilities Prior experience in a long-term care, managed health or insurance environment preferred Certified Case Manager (CCM) a plus Bilingual in English and the following language: Korean Fully vaccinated against COVID-19 In exchange, this position offers a competitive compensation and benefits package including 401k with company match, Medical, Dental, Vision, paid time off and more!
