JOB TITLE: Registered Nurse or Licensed Practical Nurse (LPN) JOB SUMMARY: The Licensed Practical Nurse shall have the personality, ability, and temperament necessary to interact with supported persons with diverse-abilities in a manner that respects and maintains the individuals' spirit, dignity, and individuality. This role is responsible for providing care and support within the scope of their nursing practice and assisting in the daily operation and leadership of the community inclusion service to promote independence, learning, and self-expression according to the abilities and choices of supported persons. REPORTS TO: Director of Service and Regional Manager KEY DUTIES & RESPONSIBILITIES: SERVICE TO SUPPORTED PERSONS Ensure the implementation, regular review, and updating of individualized program plans to address the total needs of each supported person. Communicate with fellow RNs/LPNs regarding backssments, required monitoring, necessary nursing care, and other relevant information. Update medication administration records (MAR) and medication change sheets as needed. Plan and evaluate nursing care and therapies for supported persons, collaborating with physicians, professional therapists, social workers, and other experts. Collaborate with team members without nursing backgrounds regarding medications, medication side effects, supported persons' diagnoses, and nursing care procedures, ensuring they can identify and report matters that require monitoring. Continuously backss and manage care as needed. Advocate for the active participation of each person living in the community. Provide necessary assistance and instruction to supported persons in all aspects of their daily lives. Ensure that individuals have opportunities for realistic choices in community recreational and leisure activities, tailoring these activities to their preferences.
