We are working closely alongside a Local Authority in Halton to assist with the appointment of a Reablement Worker/Care Worker, on a 3- month contract, highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration. It's essential that you have a UK Full Driving Licence. Responsibilities: To support people using services to access a range of community opportunities, including those in leisure, educational and employment services, as directed by line managers. To positively contribute to the care and support backssment and care planning process for individuals through discussion, review processes and attendance at meetings. To contribute to ongoing backssment and monitoring of people using services by maintaining and updating records, as required in line with organisational policies and procedures. To liaise and communicate with a range of stakeholders, as appropriate, to promote effective service delivery. To undertake any other duties and responsibilities as may be assigned from time to time, which are commensurate with the grade of the job. Qualifications: Attainment of NVQ 2 Experience of providing care and / or support to vulnerable adults Able to maintain written records, as required Able to communicate effectively with people using services. Able to promote independence through working practices. An understanding of and commitment to anti-discriminatory practice.
