Hope For Youth is actively hiring for a full time Program Manager at the Seaford Boys Group Home! Hope For Youth's Residential Services Department provides community based, trauma informed, youth and family driven care to adolescents and young adults in Nassau and Suffolk County. Serving youth from varying points of entry with the child welfare system, juvenile justice system, and runaway and homeless youth/young adults, the opportunities to alter the trajectory for youth in need are abundant. Youth are supported 24/7 by a team of multi-disciplinary professionals, para professionals, and agency administration that consistently meets standards for accreditation and licensing regulations.The Program Manager at the Seaford Boys Group Home has the opportunity to provide informal clinical support and case management for up to 7 youth residing in the home. Maintaining a focus on the overall mission of the agency, and the Residential Department, the social worker is charged with supporting and empowering our youth and their families to build resiliency and become self-sufficient to achieve their fullest potential. Work includes individual meetings, family and permanency team meetings, home visits, collaboration with community partners, and crisis intervention.To be a successful program manager you must be ready willing and able to do the following always including any related tasks or other agency and/or programmatic required needs associated with this role and program: Function within the policies and procedures as outlined in the program practice guide and employee manual. Provide overall management of assigned site. Complete necessary reports and case documentation. Liaise with DSS, schools and other child and youth collateral contacts. Provide crisis intervention and counseling as needed. Provide supervisees with the knowledge, skills and tools needed to succeed Adhere to state regulations and applicable code of conduct. Preparing psychosocial assessments and diagnostic evaluations Create discharge plans and provide 6 months of aftercare services Conducting home visits as needed Support other projects and residential program locations, as needed. To qualify for this job you need to have: LMSW / LCSW 2 years' post-master's experience preferred, candidates with extensive child welfare experience pre Masters will also be considered Experience with child serving systems such as DSS, probation, Foster Care Systems preferred Computer proficiency in Microsoft Office Suite, including MS Word and MS Excel A valid NYS driver's license in good standing Ability to provide 24/7 on call availability Physical Requirements: Operate a motor vehicle carrying up to ten (10) children and youth, lift/move up to fifty (50) pounds, escort children and youth into the community using stairs when necessary, restrain a child or youth during a crisis, read/write reports and log entries, walking/standing for prolonged periods, bending/stooping, kneeling/squatting, climbing stairs, reaching overhead, repetitive wrist/finger movements H ere are some of the benefits of working for Hope For Youth! • Date-of-hire eligibility for health and welfare benefits (M/D/V) • 90% Employer-paid medical premiums • Generous paid time off for all employees regularly working over 20 hours • Agency-funded Pension Plan • Pre-tax savings plans: self-directed 403(b), 529, FSA and HSA Accounts • Agency-paid life insurance and disability , and for some positions, long-term disability • Training programs and Tuition Assistance and free tuition credits are also available • Employees may be eligible for Student Loan Forgiveness Programs • Special Events and Recognition Awards • Meaningful work that makes a difference in the lives of children and families of Long Island! • Volunteer and Internship opportunities available within • Supportive like-minded, mission-driven co-workers Please include resume, cover letter and salary requirements.
