Summary The Department of Social Work and Spiritual Care seeks a part time Healthcare Equity Program Manager to join our team and support the Healthcare Equity Consult Service (HECS). The HECS Program Manager will have a combined clinical and programmatic role. They will serve as a consultant for any consults received by HECS from any of the Michigan Medicine inpatient and outpatient units.The HECS was born out of the need to mitigate concerns for bias, racism, and discrimination in care delivery at Michigan Medicine as experienced by the patients and families we serve. The HECS liaises with various departments across the Michigan Medicine enterprise to support the immediate care needs of patients and families. The HECS also identifies and addresses health system policies and procedures that may be improved to advance equitable care and care delivery access. Why Join the Department of Social Work Team? As the largest internship program in the state of Michigan, the program offers in depth and innovative social work training opportunities for MSW and BSW students from a variety of schools of social work (across Michigan and nationally). This position will assist with oversight of the program's training components and will report directly to the Director of Social Work Education. As program manager, you will interact with social work field instructors, students, schools of social work, and inter-professional leaders/colleagues. Responsibilities* The HECS Clinical Case Manager will support and work in partnership with the HECS consultants (i.e., physicians, nurses, social workers, and spiritual care providers) to serve as the consultant for referrals across the Michigan Medicine enterprise. This includes ensuring collaborative communication with the patient, family, and clinical care team members, arranging clinical team and family meetings, and requesting additional consultations for selected cases. Responsibilities include: Clinical Duties: Respond to HECS consults during business hours The Clinical Case Manager will: 1) Identify the health equity issues to be addressed and develop an engagement and response plan to address the concerns that were raised by the patient, family, or clinical care team; 2) Develop an action plan with recommendations for care in partnership with the HECS consultant team; 3) Document the recommended plan of care and actions in the patient?s electronic medical record; and 4) Communicate with the patient, family, and clinical care team members the HECS consultant?s recommended plan of care and actions. Programmatic Duties: Co-Lead Healthcare Equity Rounds: o Organizes and participates in the healthcare equity rounds at Michigan Medicine, which includes a weekly scheduled meeting o Activities may include interprofessional case discussions for early identification of healthcare equity concerns for patients, families, and clinical care team members Healthcare equity-focused presentations, facilitating debriefing sessions, individual and group education consultations Research and Scholarship Support theoretical and empirical research and scholarship related to healthcare equity, education, and training, which may include coordinating with HECS faculty and research teams in planning, implementing, organizing, and reporting project activities. May also assist with data collection and verification, data analysis, and interpretation of results. Collaborate with relevant stakeholders to develop and maintain any processes, policies, and standards to facilitate healthcare equity-focused care Participate in the education and training of interprofessional students and staff Attend regular meetings for HECS and interdisciplinary teams This position will jointly report to the leadership of Social Work and Spiritual Care Required Qualifications* Master's degree or equivalent in a field of behavioral health, social work, social science, or spiritual care For licensed disciplines - One must have a current clinical license to practice in the State of Michigan Clinical experience working with patients and families in a healthcare setting De-escalation clinical skills Direct experience with serving on a diversity, equity, and inclusion committee or related work Excellent organizational and communication (oral and written) skills Service provision with empathy, compassion, and care Efficient and accurate assessment of patient needs Exercise independent judgment with a high degree of autonomy and initiative in addressing complex situations and organizational systems Skilled in developing rapport and effectively communicating with diverse patient populations Proficiency in keeping accurate and timely clinical records and data management skills Ability to work effectively with an interdisciplinary clinical care team Desired Qualifications* Experience working on a clinical consult service Experience working with patients across the lifespan DEI facilitation and/or training experience Leading diversity, equity, and inclusion committees, initiatives, or programs. Experience or training in social science data collection methods and survey research Experience and proficiency in collecting and managing data Experience with academic publications and presentations Work Schedule This is a part time, 20 hour/week position. Schedule is to be determined, but the typical schedule will be within Monday-Friday business hours. Work Locations This position will work out of a primary work site location:1500 E. Medical Center DriveAnn Arbor, MI 48109 Background Screening Michigan Medicine conducts background screening and pre-employment drug testing on job candidates upon acceptance of a contingent job offer and may use a third party administrator to conduct background screenings. Background screenings are performed in compliance with the Fair Credit Report Act. Pre-employment drug testing applies to all selected candidates, including new or additional faculty and staff appointments, as well as transfers from other U-M campuses.
