First Choice Health Centers (FCHC) is a Federally Qualified Health Center (FQHC) seeking a podiatrist with an opportunity in East Hartford, CT! Our ideal candidate would have a strong drive to work with underserved communities, being a part of an interdisciplinary care team, and have a desire to be a part of a growing team to innovate the care delivery model. The Podiatrist would be Responsible for providing podiatric care to patients and initiate appropriate referrals. Clinical services provided shall include routine, appropriate, acute and chronic disease care as assigned and as dictated by specific training, certification and licensure. The Provider will be knowledgeable about Center and clinical policies, keep all patient information confidential, and shall assume responsibility for the development and realization of an individual ongoing care plan for all patients including the performance of selected procedures. Perform appropriate physical examination, and compile patient medical data including health history and results of physical examination. Orders or performs therapeutic procedures such as injections, and basic wound care. Instructs and counsels patients regarding compliance with prescribed therapeutic regimens, normal growth and development, emotional problems of daily living, and health maintenance as appropriate. Develops and implements patient management plans, records progress notes, and assists in provision of continuity of care. Gain competence in electronic medical record system. Completes coding and orders follow-up visit at the end of each visit. Education and Certifications: Graduate of accredited Podiatry School Connecticut License Federal and State controlled substance licenses BLS certification. Ability and Experience: Ability to identify and categorize each patient's age specific grouping of needs, such as those for child, adolescent, adult or geriatric patients. Ability to work as part of a multidisciplinary team Demonstrates flexibility and resilience in response to changing clinical or administrative environments. Demonstrates knowledge of the principles of growth and development over the life span. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served. Experience in ambulatory primary care setting preferred Experience performing therapeutic procedures Functions in a calm manner during stressful and emergency situations. Interest in serving low income-multicultural populations Possess the ability to establish and maintain collegial relationships with coworkers.
