Physical Therapist - (Part Time) Community Living

Physical Therapist SUMMARY/OBJECTIVE: The Physical Therapist is responsible for providing individuals with support and guidance in maintaining wellness that is alignment with their goals and capabilities. The PT will coordinate with house staff, primary care, occupational therapist, and Health Ops staff to provide whole-person care. Essential Functions (Job Duties): Hands on annual physical therapy assessment for each individual Attendance at annual life plan meeting for each individual as necessary. Attendance at special team meetings to address acute issues. Responding to emails from various team members. Providing in-service training to staff as needed Providing and training staff in home exercise programs, as indicated, for the people we support. Answering staff questions and concerns regarding the people we support Wheelchair responsibilities including: Meeting with vendors for wheelchair design Completing justification paperwork and coordinating completion with physician offices Wheelchair delivery and initial fitting Ongoing assessment for repair needs and adjustments Completing adjustments and repairs Modifying donated equipment to meet acute needs or the needs of new admissions. Finding a vendor and price quotes for wheelchair equipment not paid for by insurance. Assessing, organizing, and accounting for donated equipment Transfer plans including written documentation, staff training, and ongoing assessments. Ambulation plans including written documentation, staff training, and ongoing assessments. Mechanical lift responsibilities including: Equipment recommendations and selection for mechanical lifts and slings Finding a vendor and obtaining price quotes for new equipment Hands on trials of equipment with the people we support. Staff training Mechanical lift equipment repairs, including troubleshooting, finding and pricing repair parts, and installing repair parts. Shower chair responsibilities including: Equipment recommendations and selection Finding a vendor and obtaining price quotes for new equipment Hands on trials of equipment with the people we support. Shower chair repairs including troubleshooting, finding and pricing repair parts, and installing repair parts. Staff training High low beds and hospital bed responsibilities including: Equipment recommendations and selection Finding a vendor and obtaining price quotes Hands on trials of equipment with the people we support. Installation of new beds Repairs of beds including troubleshooting, finding and pricing repair parts, and installing repair parts. Competencies: 1. Communication Proficiency 2. Organization 3. Teamwork Orientation Position Type/Expected Hours of Work: This is a full-time/part-time/contractual position. Workweek schedule: M-F Hours and schedules may vary depending on Company need, location and workload. Employees will be given as much advance notice as possible when such changes occur. Required Education and Experience: 1. Master's Degree in Physical Therapy 2.
