We are seeking highly organized, proactive, compassionate individuals to join our team as Pharmacist to support our members, their families, and our clinical team. As a Pharmacist, you will play a pivotal role in collaborating with the entire healthcare team to ensure we provide exceptional primary and whole person care to Medicaid patients and ensuring the safe and effective use of medications. You will work closely with healthcare providers, members, and other staff members to provide pharmaceutical care tailored to individual member needs. Convey a positive, professional image at all times. Collaborate with clinical team in making therapeutic recommendations for appropriate medication regimens based on individual patient profiles. Provide health care team members with accurate responses to drug information questions in a timely manner utilizing available electronic and written information sources. Evaluate patient pharmacotherapy for appropriateness of drug selection, dosage, dosage form, route and duration, taking into account patient disease state(s), diagnosis, age, weight, gender, eliminating organ function, concurrent medications and allergies. Review upcoming patient appointments for evaluation of any medication issues that need to be resolved. Concisely and appropriately document all patient interactions and recommendations using EMR system or other appropriate platforms. Maintain orderly pharmacy area of acute outpatient medications and in house treatment medications ensuring all products are within expiration date; in stock based on order levels; and correctly logged and managed in the inventory management software. Ensure out of date medications are taken out of inventory and placed into secure Sharps disposal container. Monitor the need for pickup and removal and reordering replacement Sharps. Conduct Medication Therapy Management (MTM) services as presented or based on clinical criteria Other duties may be assigned. EXPECTATIONS: Ability to efficiently assess prescribed medication therapy for unnecessary medications, need for additional medications, ineffective medications, inappropriate doses, adverse drug reactions, and adherence concerns. Ability to assist with managing medication costs through cost-effective interventions. Strong clinical therapeutic skills and demonstration of knowledge of appropriate medication dosing in geriatric and adult populations Ability to efficiently evaluate and accurately document patient care notes, prior adverse reactions, patient recommendations, and prescriber recommendations in a clear, concise, professional and timely manner. Ability to convey empathy and to articulate complex information, both verbally and in writing, in a clear and concise professional manner to a variety of audiences. Exceptional Interviewing Skills Ability to probe and question patients to extract information to understand concerns and needs. Ability to provide patient centered education on chronic disease states, including medications used for prevention and treatment, and ability to help patients identify medication adherence barriers and develop solutions to overcome those barriers. Customer service focus and empathetic mindset, and commitment to member safety. Strong communication, interpersonal, and active listening skills. Attention to detail, strong organization skills, and ability to multitask. Ability to efficiently navigate approved MTM platforms and comfortable using pharmacy software systems, electronic health records (EHRs), and other technology tools to manage member information, process prescriptions, and facilitate communication with healthcare teams. The ability to work effectively as part of a multidisciplinary team, communicate openly, and share expertise is essential for success in this collaborative environment – independently and collaboratively. Cultural competency including cultural sensitivity and awareness of diversity including understanding cultural influences on medication adherence, dietary practices, and healthcare decision-making. Commitment to continuing education such as continuing education programs, pursuing advanced certifications or training in specialized areas of pharmacy, and maintaining competency about new medication and treatment guidelines. Successful completion of background check and drug screen. Maintain compliance with all Gloria Gates CARE standards, including Conduct, Compliance, and all other policies and procedures. EDUCATION AND EXPERIENCE: Pharm.D. or Bachelor’s Degree in Pharmacy from a college accredited by ACPE. Valid state pharmacy license and in good standing with the state board of pharmacy. Current license to practice Pharmacy in the state you are practicing in (PA) At least 1 year of pharmacy experience. Knowledge of delivering MTM services strongly preferred. Proficient in use of MS Office applications, including Excel, Word, and Outlook Knowledge of the Law Abide by state pharmacy law and understanding of basic compliance measures for the CMS programs, and ensure privacy and confidentiality as required by HIPAA, company and department guidelines.
