Katy Trail Community Health is seeking a Patient Care Coordinator at our Sedalia location. A Patient Care Coordinator is a member of a health care team, which includes, a provider, a behavioral health consultant, a licensed practice nurse (LPN), a registered medical assistant (RMA), a care coordinator, and a patient service representative (PSR) that have been assigned a panel of patients. The role of the patient care coordinator on the team is to provide "planned care" for the team's panel of patients who may have been identified by diagnosis or condition, as being a member of a vulnerable population, and/or as needing transition of care from an outside facility. The patient care coordinator will address barriers to care which may affect patient compliance with their care plan including following up for important appointments. This may include but not limited to, financial barriers to care, transportation assistance, language barriers, or other concerns regarding cultural appropriateness.**Katy Trail Community Health requires employees to obtain their first dose of the COVID-19 vaccination prior to their first date of employment and to be fully vaccinated by 3/15/2022. Minimum Qualifications: Demonstrate computer skills, word processingDemonstrate ability to organize and categorizeDemonstrate well developed written and oral communication skills with ability to interact with providers and third parties, including ability to utilize tact and sensitivity to timing in personal transactionsDemonstrate understanding of customer services principlesAbility to accurately record and transmit detailed informationKnowledge of available community resources and the ability to evaluate resources and make appropriate referrals preferred.Knowledge of health-related financial assistance, including Medicare and Medicaid preferred.Ability to interpret and comply with federal, state, and local laws and regulations.Ability to deal effectively with a diverse patient population.Ability to prepare reports and communicate effectively orally and in writing.Bilingual (English/Spanish/Russian) Previous experience in operation of office machinery: personal computer, copier, FAX, postage machine, printers.Adequate vision. Ability to reach, stoops, walk, and lift 25 pounds.Demonstrate Good work habits, such as attendance, punctuality, accountability, thoroughness and resourcefulness.
