PATHWAYS TRANSITIONAL LIVING PROGRAM CASE MANAGER FT The Night Ministry of Chicago, IL is looking to hire a Case Manager (at our Pathways shelter) to work with our Youth Programs. This Case Manager at Pathways TLP is a youth housing program on Chicago's West Side in the North Lawndale Community. Pathways houses up to twelve youths experiencing housing instability and homelessness and serves youth 15-24 yrs of age. The goals of the program are to provide safe and stable housing, provide life skills training, access to mental and behavioral health services, and build financial literacy and employment to prepare youth for independent living. Our program structure provides housing in two ways: Congregate and Scattered Site Apartments. The Case Manager will provide case management services to youth living in the congregate space and at two apartments leased by the agency. The case manager will develop housing and education plans, mental health assessment and safety planning, participate in resident house meetings and staff meetings, and professional development training while also providing aftercare support to former clients in need of housing-related services. From those humble beginnings over 40 years ago when Tom's first office was the trunk of his car, we have grown to a financially sound nonprofit that provides numerous award-winning services to the most vulnerable in our community. A DAY IN THE LIFE OF A CASE MANAGER AT PATHWAYS SHELTER As a Case Manager , you will promote and maintain The Night Ministry's mission to serve and empower young people and their families who are experiencing housing instability and homelessness in programs designed for youth experiencing homelessness, utilizing best practices, including harm reduction, trauma-informed care, and family strengthening. Ensure assessment, prevention, and intervention occurs in a timely and efficient manner. Help build and maintain program and community partnerships. Along with senior management, continually evaluate and develop programming to best serve youth. QUALIFICATIONS & ESSENTIAL SKILLS AND EXPERIENCE FOR A CASE MANAGER Experience providing home-based services, community outreach, and case management. Experience in the fields of child welfare, maternal/adolescent health/, and family case management is a plus. Proven ability to complete documentation promptly. Must possess strong organizational skills and excellent oral and written communication skills. Must be familiar with community-based resources especially those on and near the West Side of Chicago. Must be at ease visiting youth in their apartments, demonstrate the ability to establish productive rapport with families or client-identified support networks, and ability to handle difficult topics. Strong ability to work with school staff, counselors, social workers, and homeless liaisons. Must maintain impeccable professional boundaries. Must react quickly and demonstrate good judgment in emergencies or stressful situations. Must be attentive to group dynamics and thorough with detailed tasks and protocols. Must embrace and celebrate diversity among teammates and program participants. Awareness and endorsement of harm reduction philosophy and trauma-informed approach to services. Must be organized, attentive to deadlines, and able to creatively problem-solve. Must possess the ability to work cooperatively in a team environment and across all programs of The Night Ministry. Working knowledge of CCBYS, DCFS, and/or juvenile justice systems to work collaboratively on client cases. POSITION RESPONSIBILITIES: Provide crisis intervention, and facilitate group and individual counseling with youths and families. Provide case management services to all program participants, by assisting them with day-time and evening follow-up, case planning, and service coordination. Participate in client recruitment to include outreach at schools, community-based organizations, inter-agency programs, and follow-up upon exit from the program. Develop and maintain quality relationships with referral resources in North Lawndale and surrounding neighborhoods frequented by youth at risk of homelessness, drop-out, and housing instability. Conduct scheduled and ongoing assessments of youth needs, promote regular school attendance and conduct home and school visits. Facilitate and develop age-appropriate support and/or educational groups as needed, generally weekly. Complete Casey Life Skills Assessment, and Educational and Transition Plan at quarterly, and semi-annual time frames. Assist young adults' transition into aftercare or adult services as needed, including family reunification, transitional living, college, vocational school, military, or another safe and stable living arrangement. LGBTQ+ competency Enter program case notes and documentation in databases as required. Accurately and consistently document weekly and monthly program activities, following Night Ministry policies and requirements of funding sources. Work with volunteers and interns, including service projects, groups, and activities. Complete case management reports, and provide any anecdotal information on program impact and outcomes as requested. Participate in program audits, representing TNM to auditors and auditing agencies alongside management. Attend required agency and program orientation, initial and ongoing training, team meetings, quarterly agency All-staff meetings, training, and other meetings as required. Maintain certification in CPR, First Aid, Blood Borne Pathogens, and CPI (Non-violent Crisis Intervention) according to the schedule of training provided by the agency. Meet regularly with the supervisor for clinical and administrative supervision. Represent The Night Ministry and its client population through advocacy with other organizations, congregations, and community groups when requested; encourage community involvement to address the needs of homeless youths. Participate in fundraising, community education, community outreach, and advocacy activities and media opportunities as possible, in cooperation with The Night Ministry's Development and Must act as a representative of The Night Ministry at all times, and be an example to staff, residents, volunteers, and community members. Take initiative, and perform other duties as assigned. Essential skills and experience: Bachelor's degree in social services or a related field Excellent interpersonal skills: written and verbal Ability to work with a flexible schedule, including some evenings and weekends Must have a valid driver's license and good driving record, and access to a reliable vehicle Must pass a criminal background check Bilingual (English/Spanish) desired READY TO JOIN IN OUR FIGHT AGAINST HOMELESSNESS?
