PART TIME: COMPLETE ASSESSMENTS, PLAN of CARE and TRAIN ON PLANS OF CARE and MEDICATION REVIEWS All nursing services provided must be completed in according the New Mexico Medicaid Home and Community Based Waivers and long term care facilities, New Mexico Board of Nursing and policies, insurers, procedures and practices of Great Livin' LLC. All services provided to Great Livin' clients shall be performed according to the development of client plans of care / individual service plan and all requirements of any applicable federal and state laws, regulations, standards, and policies. The Part-time Nurse shall enter all nursing information into THERAP (State required electronic health record) as required by New Mexico Medicaid Home and Community Based Waivers and rules, regulations, laws and standards GL', LLC operates under. The Part-time Nurse reports to the Head Nurse. The Part-time Nurse can be categorized into the following general areas: 1. On-call services, phone and internet ready during on call hours. 2. Assess, monitor, and assist with Individual's health & medical needs 3. Implement Adult Nursing Services and coordinate with Head Nurse 4. Medical on-call, support to Coaches for medical decision making (PRN, med error, triage) 5. Give medications when necessary 6. Backup to Head Nurse when necessary 7. Initial employee injury assessment when appropriate Roles and Responsibilities: Assigned by RN ASSESS, MONITOR, AND ASSIST WITH INDIVIDUAL'S HEALTH & MEDICAL NEEDS · Complete Annual and change in condition assessments (echats) and complete Health Care Plans and Medical Emergency Plans. - Train and delegate HealthCare Coordinators to train DSP's Direct Service Professionals on HCP/MERP. Review of HCP and MERPs per State Standards - Completion of Nursing portion of CARMP as applicable with the Head Nurse · Review physician's / medical consultation forms and follow-up, as needed, with recommendations as applicable with Head nurse · Review of Monthly / Quarterly / Semi-Annual (as applicable) nursing reports to team · Review MARs monthly · Complete and update Therap Documentation, as appropriate, for Individuals receiving services, including, but not limited to: E-CHAT/E-CHAT Summary; MAAT; ARST; current ICD/DSM codes for medications and diagnoses with the Head Nurse · Review in Therap: - All medical appointments - Health Tracking - Medical concerns documented in T-logs or ISP Data - GERs involving Individual's health and medical issues, including, but not limited to the use of ER, urgent care, and/or EMT services; injuries; medication errors · Ensure necessary doctor's orders and/or prescriptions are present and current · Attend (back up to Head Nurse) Individual's/Great Livin' meetings as needed, to address health and medical needs, including, but not limited to: ISP/IDT meetings; Management Team meetings; Basic Assurances; HRC; Dr.'s appointments · Correspond with Teams, medical providers and/or other agencies to address Individual's health and medical issues/concerns with Head Nurse MANAGE AND IMPLEMENT ADULT NURSING SERVICES · Monitor/maintain compliance with Adult Nursing DD Waiver, Supports Waiver, and ICF-IDD standards · Monitor/consult with IDT to recommend additional and/or ongoing Adult Nursing Services as applicable (backup to Head Nurse as needed) · Create Individual Specific Adult Nursing Service deliverables (document in ISP) ((backup to Head Nurse as needed) · Provide approved Adult Nursing Services Additional Tasks · Attend Management Team and QA/QI Meetings as requested (backup to Head Nurse as needed) · Other tasks as assigned by the Great Livin' LLC Directors relevant and within the guidelines mission and ethics of Great Livin' LLC · Completion of job-related requirements per Great Livin's Policies and Procedures, and State Standards, Procedures and expectations (including - as applicable, but not limited to: completion of all required documentation before leaving shift; completion of State Abuse, Neglect and Exploitation Reporting; GERs; Time IPS) · Back up for administering medications during weekday and weekend (as needed) Knowledge and skills: · Must be able to speak, read and write in English · Proficient in use of a personal computer and various software packages · Must have or be willing to gain a working knowledge of Therap: Electronic Medical Record System · Knowledge of agency positions and responsibilities, and who to go to with specific questions and/or concerns · Must be of good integrity and possess adequate physical, mental and emotional stability so as to meet job duties in a safe and responsible manner · Able to establish priorities, work independently, and proceed with objectives without hands-on supervision · Able to handle and resolve recurring problems · Willing to work the hours necessary to meet the needs of the individuals with whom you work Experience / Educational Requirements: · Register Nurse in good standing with the New Mexico Nursing Board · Complete required trainings and certifications including, but not limited to, those mandated by the DD Waiver, Nursing Board, State and Federal Standards and Great Livin' LLC · Agreeable to additional training(s) when appropriate for the enhancement of position skills Expected Job Outcomes · Individual's health and medical needs are well addressed · Timely filing of necessary paperwork and reports · Concise and accurate completion of assigned tasks · Conduct himself/herself as a member of the Great Livin' team · Assist with the growth, quality and direction of Great Livin' · Cohesive communication between Great Livin' Head Nurse, Program Director, Operations Director, Chief Operations Officer, relevant team members related to Great Livin' and the people we support
