Part-time Geriatrician/PCP Physician

Location:
Washington, DC
Description:

Geriatrician/Primary Care Physician Role: Geriatrician/PCP, Edenbridge PACE at Skyland Location: Washington DC, Wards 7 and 8 Status: Part-time or Full-time Mission: To allow frail elderly people to age in the location of their choosing and continue to lead connected, meaningful lives Role in Brief: This physician leadership role offers a unique opportunity to help lead the first PACE (Program of All-Inclusive Care for the Elderly) program in the District of Columbia within the framework of a supportive, innovative, and expert leadership team and company. Together with our coalition of strong partners within the community, the Geriatrician will help lead Edenbridge PACE at Skyland to have a transformative impact on a particularly under-served community. This physician will have the opportunity for an adjunct academic faculty appointment at a local leading university in Washington DC, if desired. Edenbridge PACE at Skyland is aiming to be the partner of choice for District seniors and their families living in Wards 7 and 8 to allow them to live dignified, meaningful lives and to remain in their own homes for as long as they desire. Six core principles drive our approach to care. These include: We prioritize autonomy: Frail seniors deserve the dignity of respect for their choices, large or small, with personalized care plans that align with their values and desires. We value lives meaningfully lived: We focus not just on the number of days people are alive, but on the number of days they are glad to be alive. We strive to provide more of what matters. We support de-medicalization: We recognize that too much medical care is no better than too little, and that medicine is the easy answer, not always the right answer. We plant deep roots in the community: We partner with those who know and are invested in their communities, bringing our expertise and leveraging their knowledge, services, and support. We employ technology thoughtfully: Technology can enhance what we do for patients, families, and care teams—but it can never replace the human element. We focus on positive variance: We aim to recognize, understand, and replicate a person’s best days, rather than simply reacting to their worst. To help us achieve this mission, we are looking for an experienced physician who will apply PACE fundamentals to our innovative care approach, and lead the PACE clinical program, offering the opportunity to work autonomously but with the support from expert PACE operators, geriatricians and project managers. Main Responsibilities: Implementation of all clinical care across the health care delivery community, clinical outcomes (including family and patient satisfaction), and the implementation and oversight of the quality improvement (QI) program Leads the design of the PACE program’s model of care, assessment tools, risk stratification processes, and the effective implementation of the interdisciplinary team and care planning process Serves as the program’s liaison to the local, regional, and national medical and PACE communities Provides direct medical care as the PCP to program participants Supervises all medical care and services delivered through the program Evaluates and implement clinical practice guidelines based on the best available clinical evidence, while basing decisions on each participant’s goals and preferences for care, in collaboration with the interdisciplinary team (IDT) Ensures compliance with PACE provider regulations as they pertain to medical/clinical components of care Builds and maintains strong relationships with physician groups, specialists, contracted providers, hospitals, and skilled nursing/post-acute rehab facilities to strengthen these partnerships and enhance referrals. As needed, proactively address any issues related to the quality of contracted medical services Serves as a physician liaison to local, state and federal agencies, as requested Oversees the Quality Improvement (QI) program through participation on the Quality of Care Committee; design and conduct quality improvement projects and keep abreast of federal Outcomes-Based Quality Improvement initiatives. Develop yearly QI Plan, quarterly QI reports and year-end evaluation. Ensure the integrity of program data, evaluate program performance relative to national data, and implement adjustments to care plans, processes, and policies as needed to address any deficiencies Participates in national quality improvement efforts such as those organized by the National PACE Association, AMDA, and other professional groups Develops and oversees peer review systems for clinical staff, and coordinates the performance appraisal of the primary care physician. In partnership with the Executive Director, develop and maintain clinic staff job descriptions. Provides staff in-service training as well as community education and presentations in coordination with marketing as needed. Makes staffing recommendations for PACE clinical staff.
