Requisition No:824740 Agency: Department of Health Working Title: Nursing Program Specialist : 1 Position Number: Salary: 59,000:60,000 Annual Posting Closing Date: 04/03/2024 Open Competitive Opportunity Title: Nursing Program Specialist Location: Naples Salary:59,000:60,000 annual Your Specific Responsibilities: This is a professional level full:time Nursing Program Specialist with the Department of Health (DOH) in Collier County working with the Nurse:Family Partnership program (NFP). This is a grant position that will last approximately three years or until the grant is over or renewed. The NFP Nurse Home Visitor is responsible for providing comprehensive nursing services to women and their families eligible for the Nurse:Family Partnership program. Nurse home visitors are responsible for maintaining the highest standards in clinical nursing practice and adherence to the NFP model, and to policies, procedures, guidelines and standards of NFP and of the DOH:Collier County. This position reports directly to the Executive Community Health Nursing Director and to the NFP Regional Nurse Supervisor. This position requires the individual to be self:motivated, and work independently away from direct supervision. Certain immunizations such as an annual influenza vaccine are required. :Provides home visits to women and their families eligible for the NFP Program:Adheres to nursing process and the NFP model of home visitation:Carries a case load of up to 26 clients Administrative :Supports policies, procedures, guidelines and standards of NFP, the Healthy Start Coalition and the Collier County Health Department:As requested,/required, participates in community coalition in support of NFP:As requested,/required, promotes public awareness of NFP:Develops and maintains community relationships to support client referrals Clinical :Completes all required NFP education:Attends and participates in NCAST Feeding and Teaching Scales training:Develops therapeutic relationships, utilize concepts of reflection and motivational interviewing with women and their families in a home visiting environment:Performs home visiting in accordance with the NFP Model Elements and NFP Visit:to:Visit guidelines:Assesses physical, emotional, social and environmental needs of women and their families as they relate to the NFP domains:Assists women and their families in establishing goals and outcomes:Provides education, support and referral resources in assisting women and their families in attaining their targeted goals:Consults and collaborates with other professionals involved in providing services to women and families:Formulates nursing diagnosis and evaluates client progress toward stated goals and NFP outcomes:Plans home visits in accordance with client goals and NFP outcomes Communication :Maintains clear, effective, open, honest communication with both internal and external customers and colleagues:Maintains confidentiality Other : Enter EARS and services into Health Management System (HMS) according to agency protocols and timeframes: Performs accurate data input to the NFP performance management system: Stays informed of current health care developments to provide safe, quality nurse home visiting services:Attends and completes required in:services and trainings :Other duties as assigned Required Knowledge, Skills, and Abilities: : Experience in Maternal/child health, public health, or mental/behavioral nursing : Experience in conducting effective home visits : Ability to enter client data into the Health Management System (HMS): Ability to plan, organize and coordinate work assignments independently: Ability to communicate effectively, clearly and concisely through oral and written means: Ability to establish and maintain effe
