INTRODUCTION: The In Home program assists adults in maintaining themselves in their own homes despite a decrease in ability. Provides one-on-one patient interaction in a home setting. Payment is through Medicaid, VA, Long term care insurance and Private Pay. Visiting Nurse Association of Southeast Missouri is a non-profit organization.TYPE OF WORK INVOLVED: In home nursing provides weekly medication setup, Diabetic and Anticoagulant nailcare, general health assessments, check vital signs and supervision of staff performing advanced tasks in the home.TRAINING: Training involves a 30 day period where policies and procedures are covered. Computer training will cover the use of the handheld used to make patient visits. POSITION REQUIREMENTS Education: High school education or equivalent. Successful completion of an approved course in practical nursing. Certification: Currently licensed as a Licensed Practical Nurse in the state of Missouri. Experience: At least one year of experience in a health care institution functioning in the role of a L.P.N. Job Duties/Responsibilites: Provides skilled care to patients in accordance with agency policies and physician orders, and for IH clients, the Division of Senior Services Authorization. Makes pertinent nursing observations and communicates these to the Director or the physician and/or case worker when appropriate and document in a timely manner. Assists in aspects of home care as outlined by the State in carrying out the plan of treatment. Performs selected nursing procedures in those circumstances where a professional degree of evaluative judgment is not required. Participates in team and staff conferences. Attend case conferences as scheduled and documents case management with appropriate LPN. Maintains current clinical notes. Inform doctor of changes in patient's conditions, with documentation of changes in narrative notes. Review medication profile and update on handheld each visit.
