Nurse Practitioner physician job in New Jersey : Are you an experienced post-acute care nurse practitioner (NP) who aims to make a difference in the lives of your patients by providing quality care? TeamHealth needs someone like you! We are seeking a part-time nurse practitioner (NP) to work in a skilled nursing facility environment rounding on patients 3 to 4 day a week. Key Responsibilities: Perform primary care in a skilled nursing facility setting for short term and long-term patients Manage chronic disease, diagnose illness and medical conditions, and implement plan of care Order and interpret lab results Prescribe and monitor medications Consult with physicians and coordinate patient care with other healthcare providers Educate patients and their caregivers on coping skills to handle chronic and age-related conditions Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery Promote healthy lifestyle changes Desired Attributes: Critical thinking skills Excellent communication skills Ability to collaborate with the healthcare team Strong clinical skills, work ethic, and ability to work independently Adaptability and flexibility to address the fluctuations that occur during the aging process and with chronic illness Empathy, compassion and ability to connect and engage with your patients.
