We are seeking a full-time (40 hours per week) Wound Care Nurse Practitioner for float position between our Tygart Center and Pierpont Center, both located in Fairmont, WV. This role cannot support a recent graduate. The Advanced Practice Provider reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager. The Advanced Practice Provider collaborates on-site with the Center Leadership Team for center specific goals and priorities. The Advanced Practice Provider collaborates with the Medical Director and/or Attending Physician for patient specific medical care. POSITION SUMMARY: The Advanced Practice Provider provides primary and urgent care services to nursing center residents in collaboration with the other medical providers in the center. The Advanced Practice Provider works collaboratively with Center Leadership to achieve Align Med Partners mission of improving the lives we touch through the delivery of high quality health care and everyday compassion. Position HighlightsEvaluate patient health status, diagnosis, interpret labs and diagnosticsDevelop and implement patient treatment plans in collaboration with interdisciplinary care planning processConduct weekly wound rounds and document in Gehri Med according to GPS policyAssess, manage and treat wounds, identify wound etiology and cause of delayed/non-healing wounds*Address quality of life concerns related to wounds and actively develop and implement curative,maintenance, or palliative wound care goals to meet the patient/family goals of care, incorporatingadvanced care planning *Complete Genesis wound orientation, including Review of Center Framework for skin Integrity and approved Genesis Formulary productsServe as a clinical expert in wound care for nursing staff and provides targeted clinical educationMust be a graduate from an accredited School of Nursing program, licensed as a registered nurse, certified as a Nurse Practitioner *Graduate from an accredited Nurse Practitioner Program with a Master's Degree in Nursing is required.
