Job Responsibilities: Provide short-term individual counseling Crisis intervention Address funeral planning issues and transfer of responsibilities regarding fiscal, legal and health care decisions Document all findings, plans, interventions, and client progress in the clinical record Assist the patient/family in assessing financial resources when appropriate Address patient/family questions and issues Identify family dynamics and communication patterns Evaluate the patient/family response to intervention(s) when referred to community agency and satisfaction of the service(s) provided Assess the psychosocial status of clients related to terminal illness Evaluate the patient/family's response to psychosocial intervention Assesses special needs related to cultural diversity including communication, space, role of family members and special traditions Participate in team meetings Demonstrates teamwork and effective communication to accomplish client, team, and Agency goals Qualifications Masters Degree from a school of social work accredited by the Council on Social Work Education. Minimum of one year of experience in a health care setting; Home care experience preferred Excellent communication skills both written and verbal Adaptive problem solving skills CPR certification Valid driver's license, clean driving record, and reliable automobile Proof of current auto insurance Criminal background check clearance/fingerprint clearance Negative TB test
