A DAY IN THE LIFE OF A MATERNAL CHILD NURSE HOME VISITOR As a Maternal Child Health Nurse Home Visitor, you will provide individualized goal directed nursing care and education to the patients to help them make informed decisions about their babies birth and post-partum care. In addition, this position serves as a Case Manager for assigned patients. Responsibilities include but are not limited to the following: Implements (medical) plan of care, nursing interventions, and procedures as necessary to ensure the safe effective quality of care for patients. Provide health information about pregnancy, labor, and deliver. Social services including links to other human service agencies to assist with housing, food, healthcare, childcare, counselling, and employment services. Assist in managing prenatal health problems such as diabetes, asthma, cancer, and infections. Care for women after Cesarean section. Medical support for premature infants or infants with special health needs. Assistance with breastfeeding and other feeding difficulties. Education regarding preventative healthcare, childhood illness, child safety, including car restraints, postpartum depression, family planning, and other potential concerns. Stays up to date on a variety of payer sources and eligibility requirements to facilitate future billing and coverage issues as they arise. Follows up with payers as needed to ensure proper billing and continuity of care. Assesses patient/care giver needs in order to provide quality care to assigned patients. Develops and communicates a realistic plan of care for each patient. Serves as Case Manager for assigned patients and actively manages case load. Effectively fills out and completes Woonsocket program documentation, (and OASIS documentation when necessary. QUALIFICATIONS FOR A REGISTERED NURSE (RN) Registered Nurse (RN) license 2+ years of nursing experience is preferred Experience in community maternal child and family health, public health nursing preferred.
