The Visiting Nurse Association of Florida (VNA) is a 45 year old home health agency that is growing and looking to hire Licensed Practical Nurses (LPN)/ Registered Nurse in Martin/ St Lucie Counties , Florida. Multiple Cases - Adult/Pediatrics - Various Skills Needed - G-Tube/Trach/Vent Weekdays and weekends- 12 hour shifts 24/7 coverage As a Licensed Practical Nurse (LPN), you will work with referral sources from the healthcare community to implement services to meet the needs of individuals and families in their homes. You will be responsible for completing physical assessments and determine any patient home care needs, assist in developing and implementing that plan of care for each patient while using VNA of Florida's case management model. As a Registered Nurse (RN) , you will work with referral sources from the healthcare community to plan and direct services to meet the needs of individuals and families in their homes. You will be responsible for completing an initial assessment of the patient to determine home care needs, developing and implementing a plan of care for each patient while using VNA of Florida's case management model.
