HealthCare Connection is currently seeking a LPN to provide clinical care for our Northern Hamilton County locations. We are on the forefront as a model for providing quality, comprehensive healthcare in a community based center. The Lead LPN position is responsible for providing direct patient care and providing general direction to assigned nursing staff in their daily operations. Duties include but are not limited to: Determine daily assignments, pairings, and break schedules and communicate to nursing staff Assist Practice Manager and Clinical Services Coordinator in the development of the monthly clinical support schedule to ensure adequate staffing coverage Lead daily staff huddles Assist with training of nursing staff Provide direct patient care Maintain equipment and supplies and assure exam rooms are adequately stocked and equipment is properly sanitized according to established procedures Assure vaccines and crash cart are properly stocked and maintained Organize and maintain supplies in supply closet including disposal of expired or damaged supplies Conducting requested screening/tests Conducting referrals for patients Assisting providers with direct patient care including: interviewing the patient and documenting vitals and medical concerns and conditions; Assisting with Triage duties. JOB QUALIFICATIONS Education Requirements: Minimum - Licensed Practical Nurse Experience Requirements: 2 years of nursing experience preferably in ambulatory care or clinical health setting; Experience in a lead or supervisory role preferred. In addition, fluency in Spanish is a plus. SUMMARY OF CORE COMPETENCIES Demonstrated clinical skills and leadership competencies and skills for the position, including excellent communication, customer service, continuous quality improvement, relationship development, results orientation, and team building. Able to handle confidential and sensitive information while working with persons with diverse educational, socioeconomic and ethnic backgrounds. Experience and ability in entering data and maintenance of files in our electronic health records (NextGen).
