• Job Title: Licensed Practical Nurse (LPN) - Home Health Care• Location: Taylor, MI, US• Availability: Full-time and Part-time We are currently seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team in Taylor, MI. As a Home Health Nurse, you will have the opportunity to provide essential care to patients in the comfort of their own homes. Responsibilities:• Administers medications and treatments as prescribed and according to policy• Assesses patients' conditions, documents changes, and reports them to the supervisor, physician, and family• Provides direct and individualized nursing care to clients based on nursing standards and under supervision• Implements plans of care formulated by the physician and Registered Nurse• Ensures quality and safety of home health services to patients and families• Educates patients and their caregivers on pertinent information related to diagnosis and plan of care• Maintains a safe and healthy work environment• Coordinates home health services with Registered Nurses and clinical staff• Assists clients in learning appropriate self-care activities• Monitors and documents activity as required• Performs other related duties as assigned by management• Administering IVs and managing dressings to promote healing• Ensuring clear communication with the patient's medical provider• Assisting with medication administration and wound care• Providing support and guidance during the healing process• Catering to the specific needs of each patient based on their age, injury, or illness Qualifications:• Records and reports all relevant activities in accordance with the plan of care• Delivers education and information applicable to the diagnosis and plan of care to patients, clients, and their families/caregivers• Uses equipment and supplies in an effective and efficient manner• Documents and reports activities as per requirements Compensation: $35 - $45 hourly About Company: Employee input.
