First Choice Health Centers (FCHC) is a Federally Qualified Health Center (FQHC) seeking a passionate Behavioral Health Clinician, LMSW with an opportunity in East Hartford, CT! Our ideal candidate would have a strong drive to work with underserved communities, being a part of an interdisciplinary care team, and have a desire to be a part of a growing team to innovate the care delivery model. Provides direct clinical social work services, including individual, group, and family therapy. Provides minimal care coordination. Provides extensive and timely documentation. LMSW works under the supervision of a LCSW. Some evenings and weekends may be required. LMSW can only provide care to eligible Husky, private insured, and self-pay patients. Qualifications Education: Master's Degree from an accredited school of Social Work Skills: Excellent communication skills (verbal and written) Ability to use Microsoft Office and basic computer skills Professional Certification: Licensed to practice as a LMSW in CT in good standing As part of your offer acceptance, you will be required to obtain your LCSW license within 24 months of your hire date COVID-19 considerations: Employees of First Choice Health Centers must be vaccinated against COVID-19.
