SUMMARY Responsible for Hospice patients PRIMARY FUNCTIONS Universal Philosophy  Uphold expectations for safety on unit by being constantly aware of surroundings and resident.needs Maintain constant interaction with residents to provide active engagement and customer focus. Ability to redirect and engage with residents in a social and supportive manner. Possess the ability to provide creative interventions for resident social and clinical needs. Stimulate and motivate residents as often as possible. Operate under the universal worker philosophy.Role Specific Take a detailed report from off going nurse. Implementing and coordinating the delivery of care in collaboration with physician and resource.health care personnel. Providing education and information relevant to care, to patients and families as appropriate. MorseLife Health System Job Description • Interacting with other departments to ensure optimum patient care• Supervising other staff associates while on duty, and providing training and coaching in allaspects of Resnick Assisted Living Facility's policy and procedures.• Ensure teamwork through the universal worker philosophy on unit• Take ownership and responsibility for resident care, anticipate needs and take proactivemeasures.• Follow physician’s order, medication administrating policy as it applies to safe administration ofmedication and treatments.• Document medication, treatment and other pertinent finding related to patient careappropriately.• Ensuring medications and treatments are ordered in a timely manner.• Noting and transcribing physician orders for medication and treatment accurately andappropriately.• Gather all pertinent information and complete incident report appropriately, notifying physicianand responsible party.• Provide high standard of care by auditing and ensuring compliancePowered by JazzHR
