Position: Integrated Behavioral Health Coordinator Position Number: 336 O Division: Health Department: Health Reports To: Chief Behavioral Health Officer Location: Omaha Exempt/Non-Exempt: Exempt Pay Grade: Grade 10 ($24.61-37.52) Classification: Full-Time Summary: Provide clinical behavioral health services in a manner that upholds the center's mission and quality standards and results in better patient health outcome and overall satisfaction. The Behavioral Health Coordinator supports the collaborative care team, primary care providers, patient, and behavioral health department through regular consultation on an assigned caseload of patients. The Behavioral Health Coordinator advises the primary care provider on appropriate treatment strategies, provides brief interventions, educational support, and diagnosis and management of behavioral health and substance abuse conditions. Essential Responsibilities and Duties: P rovide consultations with PCPs to enhance understanding of the patient, provide decision support for treatment planning and assist in the implementation and monitoring of biopsychosocial treatment plans. Identifies and evaluates health risk behavior(s) and provides brief, targeted interventions to support optimum mental, behavioral, emotional, and physical health, in accordance with the preferred care model at PHC sites. Works cooperatively with clinic staff to provide patients with referrals for appropriate specialty Make referrals for community mental health resources as appropriate. Maintain a therapy caseload utilizing Evidence Based Treatments, including but not limited to: Functional strength-based assessment and diagnosis Psychoeducation for patients and their support systems. Medication adherence counseling and disease self-management counseling. Motivational Interviewing to develop behavioral strategies aimed at symptom reduction Brief problem-solving cognitive intervention aimed at modifying negative thinking and promoting self-efficacy. Self-Care Plan development and skills training to facilitate disease self-management, improved coping, distress tolerance, stress reduction, and relaxation Complete all charting as needed and required by contracted funding sources: including assessments, progress notes, and billing. Participate in weekly case "huddles." Participate in weekly supervision and case consultation with Behavioral Health team. Other Duties: Assist in provider training and education in integrated behavioral health-primary care model Participate in Behavioral Health quality improvement activities Participate in Behavior Health trainings and conferences. Relationships and Authority: This position has no supervisory authority. Working Environment and Physical Demands: This position is in professional office environment. Qualifications: Minimum: Master's degree in social work, or related field. Unrestricted license to practice in the state of Nebraska as a Licensed Clinical Social Worker (or working toward licensure, providing a provisional license) Competency in providing cognitive and behavioral interventions to individuals, families, couples, and groups with behavioral health in a primary care setting Excellent people skills and ability to work collaboratively with primary care staff, patients, and manage behavioral health clinicians. Valid driver's license and be insurable. Successfully pass drug and alcohol testing, motor vehicle, education, character, and criminal background checks. Preferred: 2+ years of behavioral health experience and working knowledge of Medicare, Medicaid, and health plan administration. One-year experience collaborating with Native Americans or Native American organizations. Knowledge and understanding of Public Law 93-638" Indian Self-Determination Act. Preference will be given to Ponca Tribe of Nebraska members or members of a federally recognized tribe. However, all qualified individuals are eligible.
