A DAY IN THE LIFE OF AN INTAKE NURSE/HOSPITAL LIAISON This position is responsible for processing referral orders from all sources as part of the Intake office. In addition, this position is responsible for serving as liaison between VNAHSR and area hospital to ensure smooth patient transitions from hospital to VNAHSR care . Actively reviews hospital discharge list with hospital clinical coordinators to prioritize day's work, including but not limited to, patient interviews, completion of patient referrals, coordination of required patient services, etc. Takes appropriate action to address patient needs in a timely manner. Reviews prior day's hospital admissions and discharges and checks messages to determine appropriate courses of action for VNAHSR programs (e.g. homecare, hospice, long-term care, etc.). In collaboration with various VNAHSR clinical staff, takes action to confirm patient status (e.g. physical condition, discharge/admission to hospital, etc.) in order to maximize effectiveness of start/continuance of services. Actively promotes and provides education to potential patients and their families (and RRMC Staff members) on VNAHSR services and programs to maximize opportunities to market VNAHSR services. Provides orientation sessions to new physicians regarding the scope of VNAHSR services to enhance awareness of VNAHSR programs. Accurately completes in-take referral documentation upon patient election of home care agency. Confirms accuracy of in-take referral documentation with Clinical Coordinators and other appropriate personnel at the hospital prior to transmitting documentation. Upon request, actively seeks out appropriate medical documentation (e.g. lab x-rays, patient records, etc.) to complete patient record. Reviews, data-enters and processes Face-to-Face orders in accordance with agency, State and Federal guidelines. Pro-actively advocates for patient care issues to ensure that overall quality and type of care is sensitive to each specific patient/family's needs. Records and maintains a variety of statistical data, including, but not limited to, not-taken-ups, inquires, total number of referrals, referrals to other agencies, etc. Serves as weekend Triage Nurse as assigned. Serves as homecare/hospice field nurse as directed. QUALIFICATIONS FOR AN INTAKE NURSE/HOSPITAL LIAISON Graduation from an accredited school of nursing required. Bachelor's of Science in Nursing preferred. One year of clinical experience required. Knowledge of discharge planning preferred. 1-2 years Homecare experienced preferred Valid state driver's license required. Ability to successfully pass motor vehicle record check, child abuse, criminal record check and adult abuse check required. Do you enjoy helping people? Are you able to display flexibility?
