Each year, we co-create a new story for over 220 people using a Housing First philosophy. We serve more than 50 families each year across 9 counties and 2 program sites in Idaho’s Treasure Valley. With integrated services including case management and behavioral health, CATCH's person-centered model of care offers the families that we serve individualized care, rental assistance, home visits, and stabilization supports.The Behavioral Health Therapist reports to…and works in a multi-disciplinary team of case managers, consultants, and landlord liaisons. This position will provide time-limited therapeutic interventions to clients served in order to improve psychosocial functioning. Responsibilities Supervise and advise two Behavioral Health Consultants. Facilitate a Comprehensive Biopsychosocial Evaluation and Treatment Plan that meets Idaho Medicaid criteria and requirements for documentation.Provide office-based and/or home-based mental health assessments or crisis intervention either person-to-person or through telephone or telehealth in limited rural situations. Develop a caseload, working with the Behavioral Health Consultants, of high-risk clients who would benefit from counseling during their time in the Taking Root program.Prioritize rapid access to care for clients in crisis and be available for consultation purposes with CATCH case managers, as appropriate.Utilize evidenced-based and culturally appropriate treatment strategies based on the treatment regime most appropriate for clients (such as ACT, CBT, DBT, etc.).Plan and provide mental health treatment services to patients in an individual or family setting. Manage and coordinate the reassessment of the treatment plan, which may include an adjustment of treatment intervention or goals, and transition or termination planning. Coordinate with external medication management providers while monitoring treatment adherence, including communicating with the prescribing provider any side effects and potential complications.Work within a team-based care model including participating in weekly staff meetings, managing assigned caseload, and collaborating with outside community resources/agencies. Complete and maintain timely records and reports in an integrated electronic health record regarding client’s progress, services provided, and other clinical information.Complete treatment plans consistent with Medicaid/MCO requirements to include all payor sources as applicable. Comply with all current CATCH policies, including HIPAA policies. Make appropriate referrals. Attend and participate in all staff meetings and other meetings as assigned. Portray professionalism to clients, other CATCH department personnel, referral sources, and the community during daily interactions and appearances. Adhere to company policies, practice guidelines, and professional code of ethics.Other duties, as assigned. Minimum Qualifications Knowledge and skills working with complex trauma and trauma-informed care.Successful clearance of Department of Health and Welfare criminal background check. Master's degree in Social Work, Counseling, Psychology or related field. Current professional license in the state of Idaho (LPC, LCPC, LCSW). Currently licensed through Medicaid/Optum and has experience in Medicaid billing.One year assessment and counseling experience in a behavioral health setting. Experience formulating a 5-Axis DSM IV diagnosis. Good written and verbal communication skills. Computer literate and able to meet deadlines.Experience working with disenfranchised populations. Preferred Qualifications Valid LCSW, LCPC or equivalent in the State of Idaho. Bilingual in Spanish and English Three or more years' experience assessing and counseling clients. Experience working as a member of a multidisciplinary team. Previous training and experience working with people who have experienced extensive trauma, SPMI and/or SUD populations.Previous training and experience working with people experiencing homelessness.Knowledge of community resources. Knowledge of Medication Assistance Programs. Familiarity with the State of Idaho Health & Welfare Mental Health system.
