Chorus Community Health Plan (CCHP), an affiliate of Children's Wisconsin , is an insurance organization dedicated to providing access to the highest quality health care and services to our members living in Wisconsin. We are proud to serve thousands of members throughout the state through the following product and service lines: BadgerCare Plus - CCHP is offered in twenty-eight (28) counties in eastern Wisconsin, and covers more than 150,000 adults and children, making it the second largest BadgerCare health plan in the state. With the combined resources of Children's Hospital, Children's Community Services, Children's specialty and primary care clinics, and our community partners, we are committed to improving the medical and social well-being of our members and the community. Care4Kids - Care4Kids is Wisconsin's Foster Care Medical Home and is a voluntary Medicaid benefit that provides comprehensive health care coordination services to 3,000 children who were placed in out-of-home care in Southeastern Wisconsin. The program supports the unique health needs of these children and aims to improve health care outcomes by strengthening the quality, access, and timeliness of care utilizing a "medical home" model approach for each child who enrolls in the program. Care4Kids provides children with access to primary care physicians who are trained in the needs of children in out-of-home care, and also creates a team of professionals who coordinate care for the child. Individual and Family Plans - Available for individuals and families in a 15 county service area in Wisconsin. Our Individual and Family Plans currently serve 16,000 members and offers access to high-quality health care from our broad network of providers including Children's, Ascension, Froedtert & the Medical College of Wisconsin, Bellin Health and others. Our Individual and Family Plans are available on and off the exchange providing members with comprehensive coverage and programs to meet their health care needs. Develops and maintains Access databases and related reporting functionality.Requires knowledge of computer operations techniques and/or data processing that is acquired through a high school education, plus six to twelve months of additional training in a technical school or directly related experience in an information systems environment.
