The final rate approved will typically never be at a rate equal to or above the established midpoint of the pay range for the position. Benefits: All regular full-time (RFT) status positions of 20 hours per week or more year round or part year of .50 FTE or higher are eligible for our outstanding benefits package including: Individual Medical with premium covered at 90% Free Dental insurance for the employee Employer matched retirement dollar for dollar for the first 3% Generous paid sick and vacation time 13 paid 0bserved holidays Free life and long term disability insurance Medex travel assistance Employee Assistance Program Rich and diverse mission-based working environment Position Summary : In support of the Agency's mission, this position is responsible for conducting comprehensive assessments and providing on-site crisis intervention services for low-income households that live in Washington County utilizing culturally responsive practice, guided by people with lived experience, and informed by the best practices of trauma-informed and people-centered service delivery. This involves conducting comprehensive and independent assessment interviews and intakes, establishing and documenting eligibility, and independently conducting initial intake for a variety of programs in Washington County. Assessments and referrals take place in accordance with the policies and procedures of the CARE assessment system, C0mmunity Connect Coordinated Entry system, Utility Assistance, Emergency Rent Assistance, Help Me Grow, etc. This position must accurately enter information into statewide client databases and provide additional referrals as needed. This position requires the ability to adapt to changing client needs and make independent decisions based on the presenting needs. Duties also include establishing collaborative relationships with service providers and clients while achieving positive results with diverse populations. Staff is strategically located within different Systems of care in Washington County to provide equitable and comprehensive access and assist our community as a whole. (Providence Health Clinic, Family Justice Center, ODHS) Essential Functions and Responsibilities : Conduct phone based or face-to-face comprehensive assessment interviews with low-income households that live in Washington County. Independently provide Community Action services: Rent Assistance, Utility Assistance, Community Connect Phase I Assessment, Oregon Health Plan enrollment, renewal, and navigation, TRIMET fare assistance, CARE assessment, and referrals to other CAO Programs. Allocate resources to households based on assessment results and knowledge of Community Action and county resources. Determine eligibility and initiate referrals to a variety of programs in accordance with the policies and procedures of the CARE Assessment System. Maintain real-time, accurate, complete and confidential records including paper files and entry into multiple computer record systems/databases. Submit and prepare reports as required. Maintain a comprehensive understanding of the eligibility requirements for each of Community Action's programs and community partner programs for which enrollment is regularly conducted. Coordinate and promote access to community services, provide follow-up and advocacy as needed and conduct community outreach as needed. Function independently as part of an interdisciplinary team with staff from partner agencies. Collect and upload required client documentation into the client database. Offer and provide services to individuals and families that may be outside of CAO office. This could include but is not limited to, schools, community-based organizations, a participant's home, or other agreed-upon location that provides a participant and/or family an alternative way to conduct business with the department. Attend Community Partners' staff meetings, training, and outreach events to maintain a collaborative working knowledge of the program's current statuses. Attend Community Action program meetings and trainings to maintain a working knowledge of the program's current statutes, policies, and procedures to provide effective services. As appropriate, coordinate with and provide general information pertaining to other community resources using a Community Information Exchange, Ex: Unite US Platform. Strictly adhere to all safety policies and procedures at all times and respond/correct any at-risk safety issues immediately. Strictly adhere to agency code of ethics and standards of workplace behavior. Perform other duties as assigned to meet business needs including regular and reliable attendance and adherence to all company workplace behavior standards. Marginal Functions and Responsibilities : Participate in special projects and data collection to enhance program planning and delivery. Keep informed about availability and current status of social service programs serving Washington County residents. Make client referrals as needed and coordinate with other team members to produce resource information. Other marginal functions and responsibilities as assigned. Required Education/Training/Experience: High School Diploma or equivalency with a minimum of three (3) years of experience in performing eligibility determination, including phone and face-to-face customer/client intake and delivery of information. An equivalent combination of education and experience may be considered. Bilingual in English and Spanish, both oral and written. Intermediate computer literacy in MS Word, Excel, Outlook and software programs. Experience completing data entry with a high degree of accuracy. Experience de-escalating crisis situations and providing trauma-informed client services. Experience coordinating with other professionals and social service agencies. Ability to work effectively and collaboratively in a diverse work environment. Ability to communicate effectively both orally and in writing. Ability to adapt to changing client needs and make independent decisions based on those needs. Preferred: Knowledge of Washington County community resources. Experience with computer databases, specifically OPUS and/or Service Point. Required Licensing or Other Special Certifications: N/A Required Physical Demands of Essential Functions and Responsibilities: Work is performed in normal office environment. Constant exposure to computer and keyboard with extensive phone use. In normal work day, may stand/walk 2 hrs/day; may sit _5.5_ hrs/day; may drive _1/2_ hrs/day; may use hands for repetitive grasping, pushing and pulling, typing and fine manipulation _4.5_ hrs/day; may use hands at or above shoulder level for _1_hrs/day. In normal work day, may bend, squat, and/or climb frequently. Work is light in nature, lifting maximum of 20 lbs with frequent lifting and/or carrying of objects weighing up to 20lbs. May require walking or standing to a significant degree or sitting most of the time with a degree of pushing and pulling of arms and/or leg controls.
