Summary The Housing Case Manager is responsible for housing placement and supportive services for client's approved for CCA housing programs. Duties will include but are not limited to the following: Provide Housing Placement Complete household assessment, document homelessness and develop service plan for eligible clients Housing search and placement assistance Assist with landlord negotiations and leasing guidance Provide moving and furnishing assistance, referrals and guidance Provide Supportive Services Provide assistance securing mainstream benefits (food stamps, SSI/SSDI, VA benefits, medical insurance) Provide assessment of medical and health needs and referrals for care Employment and/or vocational services assistance referrals Supportive counseling and community resource referrals as needed Document all services provided in the Homeless Management Information System within 24 hours of service delivery Any other duties assigned by supervisor Job Qualifications Minimum of a high school diploma/GED, with two years of service coordination or related social service experience required. Bachelor's degree preferred. Knowledge of homelessness and housing programs helpful Experience providing services for individuals and families experiencing homelessness Exceptional problem resolution skills Exceptional oral and written communication skills Valid LA driver's license and an insured motor vehicle in which to transport clients Job Specifications Medium: Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects 100% of the time.
