The Homeless Case Manager builds a strong collaboration with the local homeless shelters within the counties of Columbia, Dodge, Juneau, Sauk and Adams. Working directly with families to set achievable goals with target dates and empower those involved to reach their goals.The Homeless Case Manager will provide services and/or resources in the areas of: financial management, employment and training, school enrollment for children, nutrition and health, substance abuse, mental health, transportation issues, and housing placement.This position is responsible for data entry and case management notes. As well as ensuring the programs policies and procedures are met and followed. Skills Working experience in case management and/or with homeless population is helpful Effective communication skills Excellent organizational and time management skills Problem solving skills and ability to multi-task Compassionate with teamwork skills Excellent Single Coverage Benefits Package Includes: Health Dental Vision Life Paid Holidays, Sick Time and Paid Time Off Retirement contributions after 3 years of employment Position may be eligible for Student Loan Forgiveness!
