Position: Full-Time Contracted LVN for Home Health Location: Orange County Coverage Area, San Diego, CA Specialty: Home Health Shift: Day 5x8-Hour (09:00 - 18:00), flexible with On-Call rotation every 3 weeks Start Date: ASAP Job Duration: 13 weeks Renewable Observed Holidays: New Year's Day, Labor Day, Independence Day, Memorial Day, Thanksgiving, Christmas Guaranteed Weekly Hours: Yes Salary: $40/hr. We are seeking a highly motivated LVN/LPN with at least 1 year of experience in home health to join our team on a 13-week contract basis. The ideal candidate will have a valid CA license, Oasis and HCHB experience, and a flexible schedule. The LVN/LPN will be responsible for providing high-quality care to patients in the Orange County coverage area. The LVN/LPN must have reliable transportation and a valid driver's license. Description: A field position is open for a LVN/LPN for Home Health coverage in the Orange County area of San Diego, CA. This is a full-time contracted position with guaranteed weekly hours and a salary of $40/hr. The shift is from 9:00 AM to 6:00 PM with flexibility, and an on-call rotation every three weeks. The contract duration is for 13 weeks.-renewable- Responsibilities: Conduct initial and ongoing assessments of patients Develop and implement care plans based on assessments Administer medications and treatments as ordered by the physician Document patient progress and report changes in condition to the physician Provide education to patients and families on disease management, medication management, and other healthcare topics Communicate with physicians, healthcare team members, and family members Maintain accurate and up-to-date patient records in compliance with state and federal regulations Adhere to all HIPAA regulations and maintain patient confidentiality Qualifications: Valid LVN/LPN license in the state of California At least 1 year of experience in home health Oasis and HCHB experience preferred Ability to work independently and with a team Strong communication and interpersonal skills Ability to handle multiple tasks and prioritize effectively Flexible schedule Reliable transportation and a valid driver's license COVID vaccination preferred - Exemptions permitted with weekly testing Other Details: Must have a CA license in hand Minimum of 1-year Home Health experience required Oasis experience preferred HCHB experience preferred but will train. Mileage at IRS standard rate between patients.
