Health and Wellness Director – New Hope Bay POSITION DETAIL COMPANY DESCRIPTION New Hope Bay (NHB) is a licensed, all-inclusive Senior Living community providing Enhanced Independent Living, Assisted Living and Memory Care. We embrace resident-centered care; focusing on the physical, spiritual, social and emotional wellbeing of seniors. Our mission is to provide a stimulating, safe and comfortable living space for all in our home. New Hope Bay is seeking to add a dynamic and experienced Nurse (Licensed Practitioner Nurse - LPN) to our team in serving seniors at our 14-acre campus located in Hampton Township, Michigan. Through a collaborative and "person-centered" approach, the Nurse enables each resident to attain or maintain the highest practical physical, mental, and social well-being. PURPOSE OF POSITIONYour primary goal will be to maintain a quality of healthcare that ensures New Hope Bay’s resident proposition of dignified living in a nurturing home. You’ll work with the care department staff to ensure residents are cared for per the service plan. RESPONSIBILITIES - Resident Care Responsibilities include addressing all residents care needs and questions, assisting with medications, completing full clinical assessments, charting and documentation: Identifies and assesses the clinical status of potential residents Develops a personalized plan of care based on the identified physical and psychological issues of the resident including: the identified issues; resident specific goals and planned program interventions Coordinates care needs with community providers via an effective case management process Sets up a process for charting, measuring and accurately recording vital signs (temperatures, pulse, respiration, blood pressure weight and height) where appropriate Conducts new resident assessments, and assists in developing Resident Care Plan Effectively administers and participates in New Hope Bay’s “Manager on Duty” program. Trains Med passers and Caregivers RESPONSIBILITIES - Details Oversees ancillary health services as needed Demonstrates ability to orient and train staff Provides ongoing supervision of residents’ health related activities: complying with health care instructions explained by health care professionals; promptly and appropriately seeking and obtaining medical attention when needed Conducts MAR audits monthly Conducts Direct Care Partner documentation audit monthly Conducts assessments / reassessments per Benchmark standards Develop and implement the highest standards for Assisted Living services provided to residents Available by phone or pager TBD REQUIREMENTS A candidate must be skilled in directing, motivating and training staff able to organize and prioritize. Shall have the skill to communicate effectively with residents and their families and at all levels of the organization. Will react decisively and quickly in emergency situations and maintain confidentiality where required. The Nurse should be a graduate of an accredited school of nursing with a valid state LPN license and CPR certification. Two years of experience in Nursing and / or supervisory nursing experience are required.
